An exciting opportunity has arisen for a Band 3 Support Time and Recovery Worker (ST&R) to join our Assertive Outreach Team within the Community Mental Health Service.
The Mental Health and Learning Disabilities Directorate provides inpatient and community-based support to patients and carers while also supporting the wellbeing of staff.
The Community Mental Health Service interventions are provided to people living in the community who require support with severe and enduring mental health conditions.
As a Support Time and Recovery Worker (ST&R) within our Assertive Outreach Team, you will be an integral part of the Community Mental Health Service, delivering evidence-based interventions to a caseload of individuals with mental health difficulties. A collaborative, recovery-focused and strengths-based approach is essential to this role. The post holder will be supporting individuals in their community environment as well as providing in-reach to those who are in-patients to support them to return home.
The successful candidate will work within a Community Mental Health Service that has challenged the traditional boundaries between primary and secondary care in partnership with the Voluntary Community and Social Enterprise (VCSE) sector. Liaison with a wide range of partners is central to the role.
You must have a full driving licence and have permanent access to their own transport with appropriate business insurance.
Main duties of the job
Deliver recovery-focused and evidence-based interventions to individuals with severe and enduring mental health difficulties.Support service users to work towards goals identified within their care plans and promote engagement with treatment and recovery.Provide practical and emotional support to individuals within their homes and community settings.Support patients during transitions between inpatient and community services, helping to facilitate successful discharge and recovery.Develop therapeutic and professional relationships with service users, carers and families.Promote independence, wellbeing and social inclusion through person-centred support.Identify early signs of relapse or deterioration and escalate concerns appropriately to qualified team members.Work collaboratively with a wide range of partner agencies including healthcare providers, housing services, voluntary organisations and social care teams.Support service users to access services relating to accommodation, benefits, advocacy and community resources where appropriate.Maintain accurate records and contribute to service development, quality improvement and evaluation activities.Participate in supervision, team meetings and ongoing training and development opportunities.

Person Specification
Qualifications Essential
NVQ Health and Social Care or equivalent, or willing to undertake full training to achieve this standard
Experience Essential
Experience of working with people e.g in a caring situation, dealing with the general public. Experience of working in community settings. Experience of the assessment process and positive risk management.
Desirable
Previous experience in caring for people with a severe mental illness.
Additional Criteria Essential
Must be a car driver with a valid driving licence or have access to transport with appropriate business insurance in order to travel throughout the Trust, to meet the needs of the service.

How to deal sensitively with distress, disturbance and unpredictability. Knowledge of Health and Safety. Knowledge of moving and handling.
