Job summary Cheltenham Peripheral Primary Care Network is a group of 5 Practices situated around the edge of Cheltenham. We collaborate as a Network as we truly believe by working together we can achieve better and greater care for our patients in the communities. We have an established Frailty Team for the Cheltenham Peripheral PCN which is a multi-disciplinary team dedicated to supporting patients who are frail, elderly or vulnerable and living in the community. The team has been created specifically by the Cheltenham Peripheral Network to recognise the needs of the local population and the scope for transforming services to provide personalised care. The role of the frailty nurse supports CP PCNs plan to reduce the number of frail older people who have a lack of support, who want to continue living in their own homes and require access to some additional care.
