Base salary starts at $60.00/hr.

E S S E N T I A L F U N C T I O N S

• Drug Therapy monitoring for Inpatients.

• Pharmacokinetic dosing and lab monitoring.

• Antimicrobial Stewardship and Anticoagulation monitoring.

• Medication review/discharge counselling on high risk patients.

• Inpatient and post-discharge education for patients admitted with Pneumonia.

• Chemotherapy order review and management.

• Drug information resource for physicians and nurses.

• Check medication reconciliation on admissions.

• Pediatric medication monitoring.

• Morning rounds with Hospitalists.

• Medication education for providers and nursing staff.

• Therapeutic monitoring (vancomycin, warfarin management including ordering labs).

• Renal dosing.

• Apply oncology pharmacy expertise for Ivinson Memorial Hospital Cancer Center, Oncology Service Line.

• Develop and maintain status as integral team member of the medical oncology team.

• Provide services as pharmacy subject matter expert to support patients and oncology providers and staff. Including in-depth chemotherapy education for patients and caregivers.

• Integrate and collaborate with providers for appropriate treatment regimen selection and dosing/schedule.

• Ensure standardization of treatment delivery.

• Monitor and advise regarding changes in recommended treatments/regimens (utilizing NCCN), Medicare requirements, cost data changes, FDA approvals and off-label indications.

• Work with oncology clinical leadership to develop and maintain an oral chemotherapy management and tracking system.

• Develop and maintain collaborative practice agreements to optimize efficiency.

• Play a leadership role in eventual development and implementation of Epic/Beacon treatment protocols, as well as eventual implementation of VIA treatment pathways.

• Provide oncology pharmacy expertise for hazardous medication handling.

• Respond to codes.

• Work with clinical nutrition to develop nutrition plans for patients needing parenteral nutrition.

• Other duties as assigned.

E D U C A T I O N

• Graduation from an accredited school of Pharmacy required.

E X P E R I E N C E

• Three (3) years of previous hospital experience preferred.

• Board certification in any of the pharmacy specialties preferred.

• ASHP accredited residency preferred.

C R E D E N T I A L S

• Must be a Registered Pharmacist in the State of Wyoming, with a current, valid license and be in good standing with the Board of Pharmacy.

• Must be BLS certified upon hire or within 60 days of hire and must maintain certification throughout employment with IMH.

• ACLS and PALS certified within 6 months.
