Job summary Social Worker - Moving into Adulthood Team
Join Oxfordshire County Council's Moving into Adulthood Team and play a key role in supporting young people as they transition into adult social care services.
Working within a supportive and collaborative team, you will use a strengths-based approach to empower individuals to identify their goals, maximise their independence and achieve positive outcomes. You will undertake assessments, develop support plans, work closely with families and carers, and collaborate with a wide range of professionals and partner agencies to ensure young adults receive the right support at the right time.
The team is committed to delivering high-quality, person-centred practice and offers regular supervision, manageable caseloads and excellent opportunities for continued professional development. You will be encouraged to contribute to service improvement, participate in group learning and develop evidence-based practice while maintaining the highest professional standards.
We are looking for a qualified Social Worker registered with Social Work England who is passionate about supporting young people and committed to promoting independence, equality and social justice. Strong communication skills, a collaborative approach and a commitment to continuous learning are essential.

Main duties of the job As a Social Worker in the Moving into Adulthood Team, you will support young people transitioning into adult social care services through strengths-based assessments, support planning and partnership working. You will work closely with individuals, families and partner agencies to promote independence, wellbeing and positive long-term outcomes.
