Job summary
We are seeking an enthusiastic and experienced Paramedic practitioner to join our friendly and supportive GP practice team. This is an exciting opportunity to work within primary care, delivering high-quality acute same day care and supporting our dedicated home visiting service as part of a team of Paramedics.
The successful candidate will play a key role in assessing, treating, and managing patients with urgent but non-emergency clinical needs, helping to improve access to care and support continuity for our patient population.
Main duties of the job
Provide autonomous clinical assessment, diagnosis, and treatment for patients presenting with acute same day problems.Undertake home visits for housebound and vulnerable patients as part of the practice visiting team.Work collaboratively with GPs, nurses, reception staff, remote pharmacists, and the wider multidisciplinary team.Manage a varied caseload safely and effectively within scope of practice.Support care planning, admission avoidance, and appropriate onward referrals.Maintain accurate and contemporaneous clinical records using the practice clinical system.
Contribute to service development and quality improvement initiatives within the practice.

Job Summary:
The Paramedic Practitioner (PP) will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.
The post-holder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The Paramedic Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
The post-holder will use skills, knowledge and competencies as a qualified Paramedic Practitioner in order to be responsible and accountable for managing patient caseloads for treatments, referral, admission and discharging patients as appropriate.
Job responsibilities:
The post holder will provide clinical care within a team consisting of clinical lead, general practitioners, practice nurses and Healthcare assistants. Patient contact will be via patient facing clinics, triage, on-call duties and home visits Duties include, but not limited to: Care home visiting service, home visits and "minor illness" management Work as part of a multi-disciplinary team to deliver safe and high quality care. Carries out clinical practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care. Delivery of care in line with NSF, NICE guidelines and evidence-based care Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate. Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved. Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice. Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan. Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc. Assess, diagnose and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency. Refers patients to other members of the multi-disciplinary practice teams, secondary care organisations and other organisations as necessary. Works as an autonomous practitioner, in accordance with the Health Professions Council. Ensure that personal and professional clinical standards are maintained. Enhance own performance through Continuous Professional Development, imparting own knowledge and behaviours to meet the needs of the service. To achieve and demonstrate agreed standards of personal and professional development in order to meet the needs of the service. To participate in the audit process, evaluation and implementing plans and practice change in order to meet patient need. Contribute positively to the effectiveness and efficiency of the team and work colleagues.

Experience Essential
Experience of working in primary care or similar setting Experience in triage Experience of dealing with patients with undifferentiated and undiagnosed conditions Experience in triaging and managing minor illness and injury Evidence of delivering a service which improves quality and outcomes for patients Recognises priorities when problem solving, identifies deviations from the normal pattern, is able to refer to seniors or GPs Work effectively independently and as a team player.

Electronic clinical recording systems i.e. EMIS, Experience working in a primary or community care setting Telephone triage Chronic Disease Management
Qualifications Essential
Qualified paramedic or specialist paramedic - FDSc or BSc qualified and registered with HPCP or Post registration certificate (Level 3) in first contact care Chronic disease management and mental health experience
Desirable
Accredited training in chronic disease management Non-Medical Prescribing qualification to undertake the professional role of independent/supplementary non-medical prescribing
