Job summary
With the development of our therapy services, we are seeking enthusiastic and dedicated people with experience in Occupational Therapy and/or Physiotherapy to join our dynamic community rehabilitation teams at Petersfield & Bordon.
You will work within a multidisciplinary team to deliver high quality care and rehabilitation. You will use your skills and experience to support the provision of individualised rehabilitation programmes to patients with a range of functional and rehabilitative needs.
The post holder will assume responsibility for delegated aspects of healthcare delivery, working within a scope of practice under the direction/supervision of a registered practitioner.
You will have a clear understanding of patients with long term conditions and should be able to demonstrate a level clinical assessment and reasoning. Previous case holding experience and group work is desirable.
You will need to be a highly motivated individual with excellent communication skills. You will need to be confident and assertive with excellent personal skills and have the ability to demonstrate these in a pressurised environment. The successful applicant will need to have proficient IT skills.
Access to a vehicle and possession of a full driving license is essential.
There are CPD opportunities that support development of clinical skills as well as structured supervision, peer support groups and appraisals.
Main duties of the job
Work under indirect supervision from the Therapy Team Leader, Senior Therapists, and Therapists being responsible for own workload and prioritising effectively.
Assess, plan and implement Therapy to a designated caseload and adapt intervention to meet client needs:
E.g. therapeutic handling
Equipment provision
Participation in treatment activity
Dealing with distressing / sensitive issues
To ensure patients and carers are an integral part of their treatment planning and goal setting.
To be responsible for assessment, prescription and fitting of equipment, whilst demonstrating an awareness of local cost implications.
To ensure that up to date written and electronic records, reports and activity data are maintained in accordance with professional and Trust standards.
To provide intervention in a variety of environments, e.g. client's home, hospital / healthcare settings, schools, work environment to meet client needs.
To take part in procedures to ensure smooth running of the team such as patient registration, liaison reporting and recording data etc.
To adhere to national and local manual handling guidelines.
To support the work of the registered practitioners in the team. To prepare patients for and assist with clinical / therapeutic activities or treatments.
To ensure that delegated tasks and activity has been handed over clearly and are within a person's level of skill and competency.

Person Specification
Qualifications Essential
Relevant Level 5 qualification in Rehabilitation, Health or Social Care (or equivalent experience) At present, the Trust is unable to offer Skilled Worker visa sponsorship for this post. Applicants must have the right to work in the UK at the time of application.
Desirable
Working towards, or willingness to undertake once in post a level 5 Qualification. N.B, candidates who do not currently hold a Level 5 Qualification will be supported to undertake this as part of their development.
Experience Essential
Work without direct supervision. Experienced working in Physio or Occupational Therapy. Experience of applying OT or physio process. Experience of OT or Physio assessment and planning appropriate intervention. Experience of assessment and prescription of standard equipment. Ability to function as an effective member of the Community Therapy Team. Organisational skills - ability to manage variable work patterns. Ability to work under pressure, prioritise effectively and manage competing demands. Ability to problem solve and recognise own limitations. IT literate. Understanding of Health and Safety and relevant NHS policies. Understand and comply with Trusts and service standards, policies, protocols and guidelines. Team Working. Flexible and adaptive. Self-reflecting with regards to personal and professional development. Ability to secure the cooperation of colleagues at all levels across agencies. Demonstrate an acceptable level of numeracy and literacy. An awareness of risk management. Knowledge of multi-agency services available and processes for referral.
Desirable
Knowledge of local health needs strategies. Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being. Knowledge of National agendas and the effect on practice e.g. NSF's including Chronic Disease Management, and the Public Health Agenda
