Job summary
Are you passionate about supporting people with their mental health and helping them access the right support at the right time? We are looking for a compassionate and motivated Mental Health Pathway Facilitator to join Portsmouth Mental Health Hub.
In this rewarding role, you will be the first point of contact for individuals seeking mental health support. Working autonomously, you will undertake person-centred telephone assessments, identify risks, develop collaborative care plans, and connect people with appropriate mental health, housing, financial, community and wellbeing services. You will work closely with colleagues across primary care, mental health services and voluntary sector organisations to support recovery and prevent mental health crises.
We are seeking someone with excellent communication and listening skills, experience working with service users, carers or patients, and a genuine commitment to person-centred care. Knowledge of mental health and risk management is essential, as is the ability to work flexibly in a fast-paced environment.

Main duties of the job
As a Mental Health Pathway Facilitator, you will provide compassionate, person-centred support to individuals seeking help for their mental health, acting as a key point of contact within the Access to Mental Health service. You will undertake telephone-based assessments, identify areas where people need support, assess risks, and develop collaborative care plans focused on recovery and wellbeing.
The role involves signposting and referring service users to appropriate mental health, health, social care, housing, financial, community and voluntary sector services, ensuring timely access to the most suitable support. You will build therapeutic relationships, provide practical advice, support carers and families, and promote independence and recovery.
You will work closely with mental health teams, primary care providers and community organisations, ensuring effective communication and smooth transitions between services. Accurate record keeping, maintaining confidentiality, managing client information systems and documenting all contacts are essential aspects of the role.
The post holder will contribute to quality improvement, risk management, safeguarding, service development and audit activities, while maintaining professional standards and participating in training and supervision. An ability to work autonomously, communicate effectively, manage competing demands and respond sensitively to people from diverse backgrounds is essential.

Person Specification
Qualifications Essential
NVQ 3 or Health Related Qualification or equivalent experience
Desirable
Experience with working with people and patients, carers. Experience of working within a multidisciplinary team. To have a working knowledge of Risk Assessment and management tools currently in used within acute mental health settings.
