We are currently looking for four enthusiastic, compassionate and motivated Early Years Family Practitioners to join our 0-19 Public Health Nursing Service.
This is an exciting opportunity to make a real difference during the earliest years of a childs life, helping families access the right support at the right time and improving outcomes for children across Swindon.

Main duties of the job
As an Early Years Family Practitioner, you will play a key role in promoting the health, wellbeing and development of young children and their families through the delivery of the Healthy Child Programme -pre-birth to five years.
Working as part of our Health Visiting team, you will support families through both universal and targeted interventions, helping to identify needs early and ensuring appropriate support is available when it is needed most.
You will work closely with Health Visitors and other professionals to -
Build positive relationships with children and families. Deliver elements of the Healthy Child Programme. Support families to achieve positive health and wellbeing outcomes. Identify concerns and vulnerabilities at an early stage. Contribute to assessments and agreed packages of support. Work collaboratively with a range of health, education and community partners.Promote safeguarding and the welfare of children.
A significant part of this role will involve supporting the Little Beanstalks Programme, an innovative and growing early intervention initiative designed to improve developmental outcomes for children and their families.
The programme focuses on ensuring families receive timely information, advice, signposting and support at the earliest opportunity, helping them navigate services and access help.

You will be subject to an Enhanced DBS check.
You must be fluent in the English language, as required by Part 7 of the Immigration Act for the effective performance of a customer-facing role.
The ability to travel throughout the borough will be required and have access to a car.

To plan and organise activities and interventions related to the 0-19 core offer for children, young people and families under the named Health Visitor. To plan and organise home visits as appropriate as a lone worker or in conjunction with other professionals. To undertake planned development reviews. To facilitate group activities and sessions. Advise and assist parents with developing positive parenting skills and parent-child attachment. Undertake health promotion activities. Support children and their parents with school readiness. Undertake duties in clinic, early years and school settings. To maintain contemporaneous client records as per NMC guidelines and service standards. To act as an advocate for identified children and young people to facilitate access to, and understanding of appropriate services. Participate in meetings as appropriate. To be responsible for a Champion role (area of health work) and support developments within the team. The post holder must maintain confidentiality of information about children, families, staff and all health service business. Information gained must not be communicated by the post holder to other persons except in the course of duty.

BTEC level 3 Diploma in Early Years Or CACHE Diploma in Child care and Education, Level 3 (2 year course) Or NVQ/SNVQ level 3 in Early Years and Education Or NNEB Diploma
