The successful candidate will also maintain accurate clinical records and contribute to service improvement and patient safety standards.

Job description Job responsibilities

Key Responsibilities

Manage chronic disease care across QOF areas (cardiovascular, respiratory, diabetes), promoting selfmanagement and making appropriate referrals. Deliver cervical screening in line with national guidelines. Provide childhood immunisations and the full vaccination programme under PGDs/protocols. Perform Doppler assessments (ABPI) to support vascular and wound care. Undertake a full range of treatmentroom duties including BP, BMI, urinalysis, peak flow, venepuncture, wound care, ECGs and other relevant procedures. Provide evidencebased health promotion (lifestyle, diet, smoking cessation, womens and mens health, family planning). Maintain accurate electronic clinical records and contribute to audits and qualityimprovement work. Work collaboratively with nursing, clinical and administrative colleagues to deliver seamless patient care. Ensure treatment rooms are clean, wellorganised and fully stocked. Provide cover for colleagues where required. Adhere to all infection control, health & safety and confidentiality policies. Undertake any other duties appropriate to the role.

Person Specification

Qualifications Essential

Registered Nurse (NMC). Diploma level training in chronic disease management. Accredited cervical cytology screener. Competency in Doppler/ABPI assessment.

Desirable

Competency in childhood immunisations and vaccination programmes. Infection Control

Experience Essential

Minimum of one year post registration nursing experience. Experience in primary care or community nursing is preferred.

Desirable

Experience of working in primary care.

Demonstrates initiative, reliability and leadership through maintaining clinical standards. Strong organisational and planning skills; able to prioritise and work under pressure. Effective problem solving and decision making abilities. Adaptable, open to change and committed to continuous improvement. Awareness of service efficiency, patient experience and resource conscious working. Able to mentor and support colleagues and learners. Understanding of general practice operations, QOF, protocols, infection control, health & safety and risk assessment.

Desirable

Proficient in clinical IT systems such as SystmOne.
