Brockwood Medical Practice is looking for an enthusiastic and motivated Practice Nurse to join our expanding multidisciplinary team.
This is an exciting opportunity for either a newly qualified or experienced nurse who is passionate about delivering high-quality, patient-centred care in general practice and keen to develop their clinical knowledge and skills.

Main duties of the job The Practice Nurse will provide high-quality, safe and effective nursing care to patients registered with the practice. The Nurse should be able to demonstrate, appropriate to their level of training, experience and competency:

Long-Term Condition Management
Undertake assessment, monitoring and review of patients with long-term conditions, including
Diabetes
Asthma
COPD
Cardiovascular disease
Hypertension
Chronic kidney disease
Atrial fibrillation
Stroke/TIA
Other conditions as appropriate to the nurse's competencies and practice requirements
Duties may include
Clinical assessment and review
Blood pressure monitoring
Pulse and rhythm assessment
Weight and BMI monitoring
Lifestyle assessment and advice
Review of symptoms and disease control
Monitoring and review of agreed clinical parameters
Identification of deterioration and appropriate escalation
Patient education and self-management support
Appropriate onward referral to GPs or other members of the multidisciplinary team
Diabetes
Undertake diabetic reviews within competency
Perform and interpret agreed clinical measurements
Complete foot assessments
Review blood pressure, weight and lifestyle factors
Undertake appropriate blood and urine monitoring
Provide education regarding diet, exercise and self-management
Identify patients requiring GP, pharmacist, podiatry or other specialist input
Ensure appropriate coding and recall arrangements are completed
Respiratory Conditions
Support patients with asthma and COPD through
Routine clinical reviews
Assessment of symptoms and control
Inhaler technique checks
Provision of self-management advice
Smoking cessation and lifestyle advice
Appropriate monitoring and escalation
Referral to respiratory or other services where required
Cardiovascular Disease and Hypertension
Undertake blood pressure reviews
Assess cardiovascular risk factors
Support management of hypertension
Undertake cardiovascular health promotion
Monitor patients following agreed practice protocols
Escalate abnormal findings appropriately
Vaccinations and Immunisations
Undertake vaccination and immunisation programmes in accordance with national guidance, including
Childhood immunisations
Adult vaccinations
Influenza vaccination
COVID-19 vaccination where commissioned and appropriate
Pneumococcal vaccination
Shingles vaccination
Other vaccination programmes as required
Responsibilities include
Checking patient eligibility
Obtaining appropriate consent
Safe administration of vaccines
Appropriate observation and management of adverse reactions
Accurate recording and coding
Maintaining cold-chain requirement.
Stock monitoring and management
Cervical Screening
Where appropriately trained and competent
Undertake cervical screening
Obtain appropriate consent
Explain the procedure and provide patient reassurance
Ensure accurate completion and submission of samples
Maintain appropriate clinical records
Provide appropriate follow-up advice
Wound Care
Undertake assessment and management of wounds within competency, including
Acute wounds
Chronic wounds
Leg ulcers
Post-operative wounds
Skin integrity concerns
This may include
Wound assessment
Dressing selection and application
Assessment for infection
Leg ulcer assessment and management
Compression therapy where appropriately trained
Patient education and self-management advice
Appropriate referral where required
ECGs and Cardiovascular Monitoring
Undertake 12-lead ECGs where clinically indicated
Ensure accurate recording and documentation
Recognise significant abnormalities requiring escalation
Arrange appropriate GP review or onward referral
INR Monitoring
Where trained and competent
Undertake point-of-care INR testing
Record results accurately
Follow practice anticoagulation protocols
Escalate abnormal or clinically concerning results
Ensure appropriate follow-up is arranged
Phlebotomy and Clinical Investigations
Undertake venepuncture
Collect and correctly label blood samples
Undertake appropriate investigations within agreed protocols
Ensure specimens are processed and transported appropriately
Recognise and escalate abnormal or clinically concerning findings
Women's Health
Provide nursing support for women's health services within competency, including
Contraception advice and monitoring
Sexual health screening and advice
Menopause support and signposting
Cervical screening
Pregnancy-related health promotion and signposting
Appropriate referral to GPs or specialist services
Men's Health
Provide health promotion and preventative healthcare advice, including
Cardiovascular risk assessment.
Lifestyle advice.
Sexual health advice.
Appropriate signposting and referral.
Health Promotion and Prevention
Promote healthy lifestyles and preventative healthcare, including
Smoking cessation
Weight management
Healthy eating
Physical activity
Alcohol awareness

Vaccination uptake
Screening programmes
Provide appropriate advice and signpost patients to relevant NHS and community services.
NHS Health Checks
Undertake NHS Health Checks where appropriately trained and competent, including

Height and weight
BMI calculation

Health promotion and lifestyle advice

Dementia Screening
Where appropriately trained
Undertake initial dementia screening as requested by the clinical team
Complete approved cognitive assessment tools
Record findings accurately in SystmOne
Escalate concerns or abnormal findings to the GP
Provide appropriate patient and carer information and signposting
Vitamin B12 Injections
Where trained and competent
Administer Vitamin B12 injections under an appropriate PSD/PGD or other authorised prescribing arrangement
Check the patient's clinical record and treatment plan before administration
Confirm appropriate consent
Record administration accurately
Monitor and report adverse reactions
Clinical Room and Stock Management
Maintain clinical rooms to a high standard
Ensure equipment is clean, safe and appropriately maintained
Monitor clinical stock levels
Order and replenish clinical consumable.
Check expiry dates
Rotate stock appropriately
Maintain vaccine and medication storage requirements
Ensure emergency equipment is checked in accordance with practice procedures

Maintain current NMC registration

Person Specification
Qualifications Essential
Registered Nurse with current NMC registration Experience of working within a clinical healthcare environment Competent in assessment, blood pressure, venepuncture, ECGs, wound care and health promotion Good understanding of the management of common long-term conditions Understanding of safe vaccine administration and cold chain requirements Understanding of safeguarding, infection control, confidentiality and clinical governance Excellent communication and interpersonal skills Ability to prioritise workload and manage clinical appointments effectively Compassionate, professional, reliable, approachable and able to work independently and as part of a multidisciplinary team
Desirable
Previous general practice or primary care experience Experience in long-term condition management, INR monitoring and cervical screening Experience in managing a long-term condition such as diabetes, asthma, COPD, cardiovascular disease and hypertension Current immunisation and vaccination competency Experiencing using SystmOne and Ardens templates Knowledge of QOF and Primary Care Network services Experience providing patient education and behaviour change advice Experience working across multiple practice sites Interest in development
