We are looking for an enthusiastic, compassionate and highly motivated Qualified Practice Nurse to join our friendly and supportive primary care team.

The successful candidate will provide high-quality, patient-centred nursing care to our registered patient population, working both autonomously and as part of a team. The role will involve a broad range of general practice nursing duties, including long-term condition management, health promotion, immunisations, cervical screening, wound care, clinical assessments and preventative healthcare.

Main duties of the job

Main Duties:

Provide high-quality, patient-centred nursing care within the practice.

Undertake clinical assessments, treatment, monitoring and health checks.

Manage and review patients with long-term conditions (LTCs) or demonstrate a willingness to develop and progress in LTC management.

Administer vaccinations and immunisations and undertake cervical screening.

Provide wound care and other routine practice nursing procedures.

Promote health, wellbeing and preventative care through patient education and lifestyle advice.

Recognise and appropriately escalate changes in patients' health or clinical condition.

Maintain accurate clinical records and comply with practice policies, safeguarding, infection control and information governance requirements.

Work collaboratively with GPs and the wider multidisciplinary team.

Job Description Practice Nurse

Job Summary

The Practice Nurse will work as part of the primary care team within the GP surgery to provide high quality nursing care to patients. They will deliver evidence-based interventions, support patients in managing long-term conditions, provide preventative health care, and contribute to the effective running of the practice.

Key Responsibilities -Clinical Care

Assess, plan, implement, and evaluate nursing care for patients of all ages. Provide/support chronic disease management, lifestyle reviews and pre-diabetic checks. Cervical screening (smear tests) and womens health services, training provided. Provide immunisations and vaccinations (childhood, travel, influenza, shingles, etc.). Wound care, dressings, suture removal, and post-operative care. Support infection prevention and control processes. Team & Communication

Work collaboratively with GPs, HCAs, pharmacists, social prescribers, and other healthcare professionals. Actively participate in clinical meetings, and quality improvement initiatives. Provide health promotion and patient education to improve self-care and understanding of conditions. Professional & Administrative

Maintain accurate and timely patient records using the practice clinical system (EMIS). Ensure compliance with practice policies, CQC standards, and NMC Code of Conduct. Support the practice in meeting QOF (Quality and Outcomes Framework) and enhanced service targets. Contribute to infection control, cold chain, and stock management. Participate in audits, clinical governance, and appraisal.

Skills & Competencies

Registered Nurse (NMC) with current PIN. Experience in primary care or relevant clinical setting desirable. Competent in chronic disease management (or willing to learn), immunisation, wound care, and health screening. Strong communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Commitment to professional development and evidence-based practice.

Experience Essential

Essential Experience working as a Registered Nurse. Experience of delivering high-quality, patient-centred nursing care. Experience of clinical assessment, monitoring and treatment of patients. Experience of recognising and appropriately escalating changes in a patient's condition.

Desirable

Desirable Previous experience working in General Practice or Primary Care. Experience of managing and reviewing patients with long-term conditions (LTCs). Experience in areas such as diabetes, asthma, COPD and hypertension. Experience of immunisations, cervical screening and wound care. Experience of health promotion and preventative healthcare. Experience of clinical audit or quality improvement.
