Registered Nurse (PRN) Intake/Assessment Coordinator Job Type: PRN | Days Wage scale: $42.14 – $78.11 per hour This is a Union position. Position Summary The RN Intake/Assessment Coordinator conducts clinical assessments, coordinates admissions, and serves as a liaison between patients, families, referral sources, and care teams. This role supports timely access to behavioral health services while ensuring a positive patient and referral experience.

Key Responsibilities

• Complete patient assessments and referral screenings. • Gather clinical and demographic information and determine admission appropriateness. • Coordinate admissions, including insurance verification, authorizations, physician orders, and required documentation. • Communicate with patients, families, providers, and referral sources throughout the admission process. • Maintain referral tracking and support census and bed management efforts. • Serve as a patient advocate and provide ongoing support to patients and families. • Build relationships with community partners and educate referral sources on admission criteria and services. • Assist with psychosocial interventions and patient groups as needed. Qualifications Education: Associate’s Degree Nursing (ADN) Required; Bachelor’s Degree Nursing (BSN) Preferred

• Current RN license in the state of practice. • BLS and Handle With Care (HWC) certification within 45 days of hire. • Strong assessment, communication, and care coordination skills.
