The Village Surgeries is looking for a motivated and enthusiastic ANP to join our friendly Practice. We are based across two sites Croston and Eccleston.

Main duties of the job
The successfulAdvanced Nurse Practitionerwill work as an autonomous practitioner, diagnosing, managing, prescribing, and discharging patients with illness or injury across all age groups including young children. This may be by face-to-face consultation, or telephone.

Job description Job responsibilities
Assess, diagnose and manage patients presenting with undifferentiated and acute conditions.Undertake clinical examinations and request and interpret appropriate investigations.Develop and implement appropriate treatment and management plans.Prescribe safely within your scope of practice and prescribing qualification.Provide same-day and urgent care where appropriate.Recognise and appropriately manage patients who are acutely unwell, escalating to a GP or emergency services when required.Provide appropriate referrals and safety-netting advice.Maintain accurate and timely clinical records.Work collaboratively with GPs and the wider multidisciplinary team.Contribute to clinical governance, audit, quality improvement and practice development.Participate in appropriate clinical meetings, supervision and continuing professional development.Complete home visits.
Person Specification
Experience Essential
Demonstrable experience of autonomous clinical assessment, diagnosis and treatment. Excellent communication and clinical decision-making skills. Ability to recognise limitations of competence and seek appropriate advice. Experience working in General Practice. Experience of autonomous clinical assessment and decision-making. Experience managing acute and long-term conditions.
Desirable
Knowledge of EMIS. Experience leading service improvement or audits.
Qualifications Essential
Current NMC registration. Advanced Clinical Practice/Advanced Nurse Practitioner qualification or equivalent. Independent prescribing qualification. Significant post-registration clinical experience.
Desirable
Experience with chronic disease management.
