Job summary
Knoll Medical Practice is looking to recruit an enthusiastic, experienced and patient-focused Clinical Paramedic or Advanced Nurse Practitioner to join our friendly and supportive practice team.
This is an exciting opportunity for a clinician with a strong interest in acute and same-day care to play a key role in the assessment and management of patients presenting with a range of urgent and undifferentiated conditions. The role will combine face-to-face clinical consultations within the practice with daily home visits, supporting the delivery of responsive, high-quality care to our patient population.
You will work closely with our GP team and wider multidisciplinary practice team, with full clinical support available whenever required. We are committed to fostering a collaborative environment where clinicians feel valued, supported and able to develop their skills.
Main duties of the job
Assessing, diagnosing and managing patients presenting with acute, urgent and undifferentiated conditions, within your professional competence and scope of practice. Conducting face-to-face consultations in the practice, focusing primarily on acute and same-day presentations. Undertaking daily home visits for patients who are unable to attend the surgery. Recognising and managing clinical deterioration, escalating concerns and arranging appropriate onward care where necessary. Requesting and interpreting appropriate investigations and acting on results within your scope of practice. Prescribing and reviewing medication where appropriately qualified and authorised. Providing safety-netting advice and ensuring appropriate follow-up and continuity of care. Working collaboratively with GPs, practice nurses, healthcare assistants, administrative staff and other members of the wider multidisciplinary team. Maintaining accurate, timely and comprehensive clinical records in accordance with practice and professional requirements. Contributing to the ongoing development of acute-care pathways and the delivery of safe, effective and patient-centred services.

Job description Job responsibilities
To provide high-quality, patient-centred clinical assessment, diagnosis, treatment and management for patients presenting with acute and urgent medical problems within a general practice setting.
The postholder will work closely with the GP team and wider multidisciplinary team, providing safe and effective care to patients both within the practice and in their own homes. The role requires an experienced clinician who is able to work autonomously within their scope of practice, while recognising when further clinical support, escalation or referral is required.
Main Duties and Responsibilities
Clinical Responsibilities
Assess, diagnose and manage patients presenting with acute, urgent and undifferentiated medical conditions. Undertake face-to-face consultations and remote consultations where clinically appropriate. Conduct daily home visits for housebound and less mobile patients. Provide appropriate treatment, clinical advice, safety-netting and follow-up arrangements. Recognise and respond appropriately to patients who are acutely unwell or whose condition is deteriorating. Identify red flags and arrange appropriate escalation, referral or admission when required. Request, review and interpret relevant investigations within the scope of professional competence and agreed practice protocols. Prescribe safely and appropriately within the scope of practice and relevant prescribing authority, where applicable. Assess and manage common acute presentations, minor illnesses, minor injuries and acute exacerbations of long-term conditions. Provide appropriate clinical review and follow-up for patients requiring ongoing assessment. Maintain accurate, contemporaneous and comprehensive clinical records. Communicate effectively with patients, relatives, carers, colleagues and other healthcare professionals. Work collaboratively with GPs, practice nurses, healthcare assistants, PCN staff and other members of the wider multidisciplinary team. Clinical Governance and Patient Safety
Practise in accordance with professional standards, relevant legislation, local policies and practice protocols. Maintain patient confidentiality and comply with information governance requirements. Recognise and respond appropriately to safeguarding concerns. Participate in clinical audit, significant event reviews, quality improvement and other clinical governance activities. Ensure appropriate documentation, referral and follow-up of clinical care. Maintain professional registration, indemnity arrangements and continuing professional development. Work within professional competence and seek advice or support where necessary. Contribute to the safe and effective delivery of the practices acute care service. Teamwork and Service Development
Contribute positively to the development and improvement of acute care services within the practice. Work as part of a supportive, inclusive and collaborative team. Participate in practice meetings, clinical discussions, training and service development activities as appropriate. Support the effective use of digital systems and new ways of working. Person Specification
Experience Essential
Experience working as a registered Paramedic or Registered Nurse in an urgent care, emergency care, primary care or comparable clinical setting. Demonstrable experience assessing, diagnosing and managing patients presenting with acute and undifferentiated medical conditions. Experience undertaking comprehensive clinical assessments and developing appropriate management plans. Experience recognising the signs of clinical deterioration and escalating care appropriately. Experience managing clinical risk and working within professional scope of practice. Experience communicating effectively with patients, relatives, carers and multidisciplinary colleagues. Experience maintaining accurate and contemporaneous clinical records. Experience working autonomously while seeking advice or support when required. Experience prioritising and managing a varied clinical workload. Experience working collaboratively as part of a multidisciplinary team. Experience undertaking or willingness to undertake patient home visits.
Desirable
Previous experience working in a general practice or primary care setting. Experience managing same-day or urgent care presentations within general practice. Experience undertaking home visits and assessing housebound patients. Experience managing acute exacerbations of long-term conditions, such as asthma, COPD, diabetes or cardiovascular disease. Experience assessing and managing minor illness and minor injuries. Experience requesting, interpreting and acting upon clinical investigations, including blood tests and ECGs. Experience prescribing or working within a prescribing-led clinical service. Experience using EMIS Web or another primary care clinical system. Experience working with multidisciplinary teams, community services and secondary care providers. Experience contributing to clinical audit, significant event reviews, service development or quality improvement. Experience supporting or mentoring students, trainees or other members of the clinical team. Experience implementing new clinical pathways, digital systems or service improvements.
Qualifications Essential
Health and Care Professions Council (HCPC) registration as a Paramedic; or Nursing and Midwifery Council (NMC) registration as a Registered Nurse. Relevant clinical experience in urgent care, emergency care, primary care or a comparable clinical environment. Evidence of continuing professional development relevant to acute and urgent clinical assessment. Appropriate professional indemnity arrangements.
Desirable
Postgraduate qualification in Advanced Clinical Practice or equivalent advanced clinical training. Independent prescribing qualification. Relevant minor illness, minor injury or acute assessment qualifications. Immediate Life Support or Advanced Life Support certification. Additional postgraduate qualifications relevant to primary, urgent or emergency care.
