Job summary
Stone & Eccleshall Primary Care Network is seeking a compassionate and motivated Wellbeing Practitioner / Social Prescribing Link Worker to join our multidisciplinary team. This is an exciting opportunity to help people access the support they need beyond traditional medical care, addressing the social, emotional and practical factors that impact health and wellbeing. Working across our PCN practices, you will empower individuals to take control of their wellbeing through personalised support and connections to local community services, voluntary organisations and statutory agencies.
Main duties of the job
Work directly with patients referred from GP practices and community partners.
Conduct person-centred wellbeing conversations and assessments.
Develop personalised support plans focused on what matters most to each individual.
Help people access community groups, social activities and practical support services.
Support individuals experiencing issues such as loneliness, financial difficulties, unemployment, housing concerns and emotional wellbeing challenges.
Build strong relationships with local voluntary, community and statutory organisations.
Maintain accurate records and contribute to service improvement initiatives.
Work collaboratively with GPs, practice teams and wider PCN colleagues

Job description Job responsibilities
The Wellbeing Practitioner / Social Prescribing Link Worker plays a key role in empowering people to take control of their health and wellbeing through personalised, non-medical support. Working within Stone & Eccleshall Primary Care Network, the post holder will provide holistic, person-centred guidance, connecting individuals to community groups, voluntary sector organisations and statutory services that can support their wider social, emotional and everyday living needs. The role requires close collaboration with the network practices, PCN colleagues and a wide range of local partners. The postholder will be expected to work across the three network practices on a rota basis and will manage a varied caseload, with face to face appointments and development of personalised support plans. They will also support the development and sustainability of local VCSE provision and community groups.

Qualifications Essential
Qualifications GSCE grade 1 in English & Maths (or equivalent) Level 3 qualification in health, social care, community development or a related field Training in motivational coaching & interviewing or equivalent experience
Desirable
Customer/patient care experience gained from a minimum of 1 year in a general practice or healthcare environment Experience of supporting people with their mental health needs, either in a paid, unpaid or informal capacity Experience of partnership/collaborative working and of building relationships across a variety of organisations Ability to work flexibly and enthusiastically within a team or on own initiative Ability to recognise when a persons needs fall outside the scope of the role and to refer appropriately to qualified health professionals or other agencies, including for mental health concerns.

Knowledge of the personalised care approach. Utilises the evidence base for social prescribing interventions and activities. Be able to think ahead and identify potential problems Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers High level of written and oral communication skills Experience working with EMIS/DOCMAN would be advantageous Competency in working with MS Office packages (Outlook, Word, Excel) Excellent organisational skills Be able to work independently, without direct supervision and within your scope of practice Be able to manage time, tasks and documentation efficiently Understand when to escalate, refer and seek support
