We are looking for an enthusiastic and motivated individual to join our successful team as a Cancer Pathway Tracker. The post holder will proactively track patients as they move through the Trust and comply with National Waiting Times Guidance and Standards.
The successful candidate must demonstrate effective communication, be flexible, a team player and have the ability to work on their own initiative.

Main duties of the job
To support the clinical teams with compliance of targets noted in the Cancer Plan and the Manual of Quality Measures for Cancer Peer Review.To establish and develop communications with other colleagues in neighbouring and tertiary trusts who are involved in patients care pathways.To collect, collate and input all data required for submission to the National Cancer Waiting Times database for the Trust's Cancer Target Monitoring as per national guidelines.To facilitate smooth and efficient patient pathways for all cancer patients, by utilising the cancer tracking system and working with clinical teams.To assist clinical teams in the collection of evidence required to demonstrate compliance withthe Manual of Quality Measures for the Trust's Cancer Peer Review.

To proactively track individual patients as they move through the Trust from referral to treatment, ensuring that their pathways comply with national waiting times guidance and standards. To engage with clinical teams to ensure that all efforts are made to avoid breaches of waiting times standards. To engage with waiting times colleagues at neighbouring and tertiary trusts to ensure that patients referred elsewhere still begin treatment within waiting times target times. To collate data from various sources to ensure that patient pathway information is up to date on the Trust cancer tracking system, especially prior to MDT meetings. Work with immediate colleagues and clinical teams to identify new data sources and develop new processes for improving data quality
