Job summary
Slough OPMH has an exciting opportunity for a Community Psychiatric Nurse who wishes to join our dynamic Home Treatment Crisis team. A flexible working shift pattern is just one of the attractions this opportunity will provide to you. You will provide high quality treatments and will work with the clinical team to take responsibility for the assessment and implementation of intensive care and treatment people with mental health needs/dementia in the community.
You must have specialist knowledge of the care of older people with mental health problems and mental health crises. You will be required to travel within the community for the performance of your duties.

Main duties of the job
Participating as a member of a multi-disciplinary team managing a caseload of patients To undertake assessments of patients, involving their carers and family members in the patient's care and care plans. To be responsible for the formulation, execution, and evaluation of care plans for a designated number of patients who may present with significant complexity, in consultation with the patient and the multi-disciplinary team. The post holder will direct and supervise Support Workers and/or students. Post holder will provide advice to other members of the service and agencies on the needs of service users. To provide a fast and flexible response to care needs, responding to referrals in a timely manner and providing intensive treatment (up to four times daily). Education, qualifications or training Essential
Registered Mental Health Nurse NMC Registration Specialist knowledge acquired through post graduate short courses, training and equivalent experience, in a relevant field.

Previous Experience Essential
Demonstrable experience of working autonomously to deliver high quality and evidence-based nursing care. Experience in providing supervision to junior colleagues and students in the completion of routine nursing and clinical skills. Knowledge, skills and abilities Essential
Demonstrable understanding of an acute mental health environment and relevant legislation such as the Mental Health Act, Mental Capacity Act and DOLs Proficient use of digital tools and applications including Word, Outlook and patient management systems.
Additional Requirements Essential
Ability to travel between multiple Trust sites and non-Trust locations, including patient homes, some of which may not be easily accessible via public transport.
