About the role This is a permanent part-time Clinical Partner - Nursing role and looks after clients living in the Central West/Nepean/Blue Mountains Region. This is 3 days per week between Monday - Friday.
HammondCare are seeking 3 x 0.6FTE Registered Nurses to cover a different area each:
• Penrith, Hawkesbury & Lower Blue Mountains
• Upper Blue Mountains
• Central West
As Clinical Partner – Registered Nurse, you’ll be a valued member of our growing Home Care Clinical Care & Practice Team, working closely with an experienced, multidisciplinary team to provide tailored and individualised services to older adults living at home.
A typical day will involve assessing functional care needs, setting care goals, and providing treatment and intervention, helping people stay active, independent and right where they belong – home.
You’ll be provided with a broad and varied caseload, with support from experienced clinicians. This meaningful role will include performing home visits or running group activities for clients accessing Support at Home or CHSP services or residing in one of our Seniors Living units. You will deliver person-centred care in an environment that promotes continuous quality improvement, consistent with the values of HammondCare and the Australian Health Practitioners Regulatory Agency (AHPRA).

About you
• You’re a Registered Nurse with current AHPRA registration and practical experience, or sound knowledge in Aged Care, Inpatient, Community, Home-Based or Rehabilitation or related setting.
• You're a strong communicator, and able to plan and manage your time effectively.
• You enjoy working independently as part of a multidisciplinary team.
• You have access to a reliable vehicle, valid driver’s license and safe driving record to travel between client’s homes and HammondCare locations.
