Job Description: Essentia Health is hiring the Administrator for our senior living campus in Detroit Lakes, Minnesota: Oak Crossing, an 84-bed skilled nursing facility, together with Lincoln Park Assisted Living and the Lincoln Park and Winchester senior apartments. That is about 172 beds and apartments, roughly 190 team members, and four buildings within minutes of each other, all part of an integrated health system with the hospital and clinics right next door. It is the most senior post-acute care leadership role in our West Market, reporting to the VP Administrator for Detroit Lakes and working closely with Essentia's System Vice President for Post-Acute Care. This is a campus that works. Oak Crossing holds a five-star overall rating from CMS, with five stars for staffing and turnover well below the norm for rural long-term care. We are looking for an experienced long-term care leader who will keep it that way: someone who runs a tight, compliant, resident-centered operation, develops the leaders around them, and wants the backing of a system and works well inside one.
Education Qualifications: Bachelor's degree in health care administration, business, nursing, or a related field
What You Will Do:
• Lead day-to-day operations across skilled nursing, assisted living, and senior housing: census and occupancy, staffing, quality and safety, resident and family experience, and financial performance. • Serve as the licensed Nursing Home Administrator of record for Oak Crossing, and hold or work toward the Assisted Living Director license for the campus. • Own survey readiness and regulatory compliance with CMS, the Minnesota Department of Health, and Minnesota assisted living licensure under Chapter 144G, and lead the campus QAPI program. • Develop and lead the campus leadership team, including the Director of Nursing and the Assisted Living Manager, and build a culture that people stay in. • Manage operating and capital budgets, performance to budget, and staffing to metrics. • Be the face of the campus in Detroit Lakes: residents, families, referral partners, the medical staff next door, and the community. • Work as a partner with Essentia post-acute and market leadership: bring issues forward early, share the data, and align the campus with system priorities.
What You Will Need:
• Current Minnesota Nursing Home Administrator license, or eligibility to obtain one by endorsement before your start date. • Three or more years in a leadership role in skilled nursing, assisted living, or senior living, with direct responsibility for regulatory compliance and survey outcomes. • Working knowledge of the CMS Requirements of Participation, the Minnesota Department of Health survey process, and Minnesota Chapter 144G. • Financial acumen: budgets, census management, labor management, and the ability to explain the numbers to your team. • A collaborative, transparent leadership style. You are comfortable being asked questions about your operation and you treat system leaders as partners.
What Makes You a Strong Fit:
• Minnesota Licensed Assisted Living Director or Licensed Health Services Executive. • Master's degree in health care administration, business, or a related field. • Multi-site or campus leadership spanning skilled nursing, assisted living, and senior housing. • Experience inside an integrated health system or a nursing home connected to a hospital. • A record of strong survey results and low staff turnover. • Roots in rural Minnesota or the Upper Midwest, or a real desire to put them down.
Work Location:
• This role is on-site in Detroit Lakes, Minnesota, across the four campus buildings. Detroit Lakes is lakes country, about 45 minutes east of Fargo-Moorhead.
Licensure/Certification Qualifications: Current Minnesota Nursing Home Administrator license required, or eligibility for licensure by endorsement prior to start. Minnesota Licensed Assisted Living Director or Licensed Health Services Executive preferred.
