Overview:

Provides medical case management services under the Kentucky HIV/AIDS Care Coordination Program (KHCCP), including social service assessments and connecting HIV+ individuals with needed services offered by Federal, State, and Local community programs.

Key Responsibilities

• Conduct nutrition assessments with clients and identify nutritional diagnoses.

• Meet with patients who have nutritional needs based on their clinical nutritional assessment and provide counseling, suggestions, and resources.

• Develop individualized plans and goals for nutrition interventions and therapies for clients.

• Promote better nutrition by speaking to clients about diet, nutrition, and the relationship between good eating habits and managing HIV/AIDS as well as other related health issues.

• Evaluate the effectiveness and measure progress toward outcomes of medical nutrition therapy interventions and make changes as indicated

• Will utilize assessment and evaluation techniques that consider the varied needs of age-specific populations, as well as cultural, religious, and ethnic concerns

QUALIFICATIONS:

• Bachelor’s degree in human nutrition, food and nutrition, dietetics, food systems management, or an equivalent major course of study.

• Registered Dietitian (RD) through the Commission on Dietitian Registration (CDR) of the American Dietetic Association (ADA).

• Licensure as a dietitian with the Kentucky and Indiana Board of Licensure and Certification for Dietitians and Nutritionists. Experience working in a nonprofit organization or a strong desire to work in a service-driven environment.
