Job summary
This is a part time role, working 15 hours per week. You will be based at Whaddon Healthcare, with some clinics off site. This role involves working in the community.
Main duties of the job
To manage a caseload of patients with complex Heart Failure, using evidence-based practice and patient centred goals to assess, plan, implement and evaluate interventions.
To support the Heart Failure Advanced Nurse Practitioners (HF ANP) with titration clinics, undertaking basic assessments to support clinical decision making for medicines optimisation.
To work as part of the multidisciplinary team with the aim of preventing admission, supporting effective discharge, and providing ongoing care for patients with Chronic Heart Failure within the community.
To undertake a key worker role, co-ordinating multidisciplinary care as appropriate.
To provide expert support, advice, and education to raise awareness of the needs of patients with Chronic Heart Failure.
To undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues
The post holder will be an active team participant and willing to undergo the required training and development to support succession planning within the service

It is a condition of your employment that you are currently registered with the NMC and it is your responsibility to maintain your professional registration.
Act as a role model in expert practice facilitating and supporting the delivery of effective patient care using innovation and engagement to promote best possible outcomes.
Plan and develop appropriate management care plans for individual patients or client groups, with guidance and support from the HF ANPs.
Identify and promote areas for practice development with the HF ANP, including guidelines and procedures to support the delivery of care, ensuring that all care is evidenced based.
To be professionally and legally accountable for all aspects of own work including management of patients in your care.
To organise the caseload efficiently and effectively with regard to clinical priorities and use of time.
To communicate effectively and work collaboratively with multidisciplinary team to ensure co-ordinated service.
To maintain patient records in line with the practice Record Keeping policy.
Provide advice to patients and professionals and liaise with the community services for provision of palliative care packages.
Promote better understanding of cardiac conditions for patients, their families and carers, and other agencies involved with the patients through education, support, and counselling.
Provide patients and their families/carers with information on standards they should expect from the team.
Ensure that standards of care and treatment are maintained, in line with National Guidance (NICE) European Guidelines, local guidelines and policies.
Responsible for the effective use of resources.
Responsible for safe use of equipment.
Deal with complaints in a positive and sensitive way in accordance with practice policy
Attend Home Visits to help patients within the community
Person Specification
Qualifications Essential
Registered Nurse Post registration qualification in heart failure Nurse Prescribing (or prepared to undertake training)
Experience Essential
Post registration experience, working either in the community or acute setting Experienced and competent in the management of patients with heart failure
Desirable
Knowledge of community services and benefits of cross organisational working
