About us
Role Purpose
This is a frailty-led paramedic role within an innovative single practice Primary Care Network, focused on delivering proactive, preventative and patient-centred care to frail and vulnerable patients, particularly those in care homes or at risk of deterioration.
The postholder will play a central role in reducing avoidable hospital admissions, supporting patients to remain in their usual place of residence and delivering anticipatory and coordinated care across the system.
Alongside this, the role will contribute to acute and same-day demand, providing responsive clinical assessment and triage for the wider practice population.
The role requires a clinician who is comfortable working autonomously, while also contributing meaningfully within a multidisciplinary team environment.
Foundry is made up of 4 sites and has over 29,000 patients across our practices. We have many teams that help provide the most effective treatment to our patients and can support our GPs.
Job description Job responsibilities
1. Frailty & Proactive Care (Primary Focus)
Lead on the identification, assessment, and management of frailty
Deliver holistic, person-centred assessments including physical, psychological, and social needs
Develop and implement personalised care and support plans, including anticipatory and end-of-life care planning
Manage a defined caseload of frail and complex patients, including those with dementia
Support admission avoidance and early intervention strategies
Work closely with MDT colleagues to coordinate care and improve outcomes
2. Care Home & Domiciliary Care
Provide regular clinical reviews and acute assessments for care home residents
Undertake home visits for housebound and vulnerable patients
Support care home staff with clinical decision-making and escalation
Contribute to care home MDT meetings and ward rounds
Support medication optimisation and care planning alongside pharmacy and nursing teams
3. Acute & Same-Day Care
Undertake urgent clinical assessments, including triage and same-day demand
Assess and manage minor illness, injury, and exacerbations of long-term conditions
Coordinate and deliver telephone triage and clinical prioritisation
Escalate appropriately to GP or secondary care whenrequired
4. Clinical Practice & Procedures
Conduct autonomous assessment, diagnosis, and treatment within scope of practice
Request and interpret diagnostic tests (including ECGs where trained)
Prescribe medication (where qualified) in line with PGDs and clinical competence
Refer patients appropriately to secondary care and community services
5. MDT & System Working
Work collaboratively with GPs, nurses, pharmacists, socialprescribersand wider MDT
Participate in frailty MDT meetings, case reviews, and service development
Liaise effectively with hospital, community, and voluntary sector services
6. Quality, Governance & Professional Practice
Maintainaccurate, contemporaneous clinical records
Work in line with HCPC standards and College of Paramedics guidelines
Participate in audit, quality improvement, and service development
Adhere to confidentiality, GDPR, safeguarding, and information governance standards

Qualifications Essential
Registered Paramedic with Health and Care Professions Council (HCPC) Degree in Paramedic Science or equivalent Minimum 2 years post-registration experience Driving licence and access to a vehicle

MSc or working towards Advanced Clinical Practice Non-Medical Prescribing qualification Membership of College of Paramedics Advanced Clinical Practitioner status
