Job summary

An exciting opportunity to work as an Anticoagulation Nurse Practitioner in Staffordshire Thrombosis and Anticoagulation Centre (STAC) delivering high quality care for patients on an anticoagulant. STAC is specialised centre with a multi disciplinary team across Haematology, Thrombosis and Oncology. We are looking for a highly motivated individual who strives for excellence and embraces change and service improvement.

Main duties of the job

Provide direct clinical care through health assessment, planning and delivery of appropriate treatment, monitoring and follow up for patients requiring anticoagulation.

Lead development and review of guidelines, patient pathways, protocols and training events.

Prepare and present multi disciplinary team meetings.

Job description Job responsibilities

Provide direct clinical care through health assessment, planning and delivery of appropriate treatment, monitoring and follow up for patients requiring anticoagulation, thrombosis, cancer and other haematology conditions.

To establish, deliver and promote safe and effective care pathways.

Lead development and review of guidelines, patient pathways, protocols and training resources for clinical care, patient and staff education.

To develop evidence based local protocols for prevention of thrombosis, prevention and management of risks of anticoagulation and safe and effective anticoagulation in special situations.

To provide expert clinical advice to patients and other clinicians. Plan and deliver care for patients with complex thrombosis and haematology problems in acute emergency portals, inpatients, day unit and outpatients setting.

To help prepare, present and action outcomes of multi-disciplinary team meetings.

To develop and promote robust patient safety practices ensuring safe and effective anticoagulation of patients.

To promote clinical practice through research and education.

Lead improvement in care provisions by data collection, audits, quality assurances and improvement initiatives and assessment of clinical outcomes.

Actively participate in department, directorate and organisational clinical governance structure.

To work with other clinicians in developing robust clinical governance structure including reporting, review and analysis of adverse events, near misses and develop strategies to minimise risk and potential harm.

The post holder will ensure safe and effective continuity of care to cover annual leave and other absences with the other anticoagulation nurse practitioner, clinical nurse specialists and the advanced clinical practitioners by arranging appropriate reciprocal prospective cover.

The post holder will continue to maintain and update their knowledge and skills in anticoagulation and haematological disorders through continuing professional development.

Person Specification

Qualifications Essential

Current registration with professional body such as NMC Registered Independent Non-medical Prescriber Evidence of study at level 6. ALS/ILS trained

Desirable

Mentor and or teaching qualification Evidence of study at level 7

Knowledge, skills and Experience Essential

Evidence of continued professional development Experience across a wide range of clinical area that is underpinned by theory and continued knowledge and skills through CPD, including at least two years anticoagulation experience Evidence of involvement in policy and protocol development and implementation and ability to influence service changes Comprehensive knowledge of thrombosis and anticoagulation management, prescribing different anticoagulants and detailed understanding of risks, side effects and cautions. Ability to work within a multi-professional team Ability to adapt communication methods to a wide variety of patient abilities/needs Accepts constructive feedback and teaching from a variety of sources Deals effectively with clinical issues with defined procedures and guidelines Self-motivated with evidence of efficient time and deadline management Able to identify learning opportunities and able to organise own learning and development Able to undertake required clinical duties/procedures Ability to undertake computer and other equipment skills

Management and leadership experience Clinically working in an autonomous role Experience of service development Evidence of multidisciplinary teaching
