Main duties of the job As a Team Leader, you will play a crucial role in assisting the home management team to provide exceptional care at the Chertsey Parklands Care Home. Your responsibilities include leading personnel in care planning, evaluating care plans monthly, conducting audits, and implementing action plans. Additionally, you'll aid the transition process for residents being discharged from the home while monitoring their ongoing well-being. You'll manage safe work practices and ensure both personal safety and the safety of those around you. The position requires strong communication skills, the ability to lead in the manager's absence, and compliance with Avery's values.

Shift Details: Dementia Unit, shift hours 7:00-19:30, two weeks rolling rota, one week 3 shifts, one week 4 shifts, working every other weekend

ABOUT THE ROLE
Your focus asTeamLeaderwill be tosupportthehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open,supportive, andinclusiveenvironment for residents, visitors,and staff.
Other responsibilitieswillinclude:
Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans. Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate. Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation. Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.

Our ideal candidatemust:
Have anNVQ 3 or equivalent in care for care homes. Two years experience working in a senior position with the resident group. Have the ability tocommunicate effectively both verbally and in writing. Demonstrate the ability to lead staff and act responsibly in the absence of the manager.

Person Specification
Qualifications Essential
Our ideal candidate must have an NVQ 3 or equivalent in care, with at least two years of senior experience in a similar role. Communication skills, both verbal and written, are essential, as is the ability to lead staff effectively and responsibly, especially in the manager's absence. Adhering to Avery's values of being caring, supportive, honest, respectful, and accountable is paramount for success in this role.
