The Home Health Physical Therapist will provide skilled physical therapy services in accordance with each patient's individualized plan of care, including initial and ongoing assessment of progress and response to treatment. Accurately document evaluations, interventions, and patient outcomes while collaborating with the care team to support quality patient care. Consistently model Westminster-Canterbury's core values of Respect, Passion, and Professionalism and hold others accountable to the same standards.
What You'll Do In This Role
• Provide services in accordance with accepted standards of practice for physical therapy.
• Document all therapy interventions.
• Provide initial and ongoing assessment and evaluation of patient/family response to therapy
• Develop and revise the plan of care in consultation with the physician and other care-team members.
• Provide therapeutic treatments and evaluation of equipment needs as applicable.
• Advise and consult with the family and other agency personnel as applicable.
• Complete documentation timely for all assessment and evaluation data, treatments, and patient's response to therapy intervention.
• Evaluate outcomes of care and complete the OASIS assessment accurately.
• Provide discharge planning and evaluation.
• Supervise physical therapy assistants at least every 30 days and home health aides every 14 days when appropriate.
• Complete all training and in-service programs as assigned.
• Performs other duties as requested.

Physical Requirements
• Must be able to climb at least 2 flights of stairs
• Stand/or walk for a period of at least 4 hours
The Experience You'll Bring
Required
• Current Virginia Physical Therapist License AND one of the following:
• Education: Graduation from a physical therapist education program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE) or a CAPTE successor organization. Internationally educated candidates must have completed a program determined to be substantially equivalent to U.S. entry-level physical therapist education by an approved credentials evaluation organization.
OR
• Examination: Successful completion of a state-approved physical therapist examination in the state where physical therapy services are provided.
• At least one year of professional experience
• Experience with Microsoft Office and Google Suite
• Valid state driver's license; Motor Vehicle Driving Record satisfactory to Westminster and its insurance providers
• Proof of motor vehicle insurance
• Flu-Vaccinated
Preferred
• Experience working with an elderly population in Home Health
Compensation & Benefits
• Compensation Range: $48.26 - $60.32 hourly (based on relevant experience)
