Patient Navigator-Emergency Services
An Emergency Department Patient Navigator is a trained, culturally sensitive health care worker capable of providing support and guidance helping patients transition from the emergency department to outpatient follow-up care or home. Provide support and guidance throughout the hospital and community. Help navigate doctors' offices, clinics, hospitals, outpatient centers, insurance and payment systems, and other components.

• Assisting with SBIRT Screening
• MH Care Coordination
• Monitoring and following up on discharge call backs
• Coordinating appointments with providers to ensure timely delivery of diagnostic and treatment services.
Qualifications:
• High School Graduate or GED
• 2 years experience in healthcare, hospital or clinic environment.
• Experience in customer relations preferred
• Ability to organize and manage a large volume of incoming documents accurately with minimum supervision
• Ability to work cooperatively with others
• Capable of data entry and productivity utilizing Microsoft Office Suite (Word, Excel, PP, etc)
• Detail oriented
• Able to work well under heavy workload
• Optimistic, energetic and outgoing
