In OOH we assess and treat patients who require urgent unscheduled intervention suitable for treatment by general practice clinicians when the GP surgeries are closed. Our referrals come from either 111, via a dedicated health care professional line used by paramedics and district nursing teams or from the AED or MIU. Main duties of the job Providing comprehensive clinical assessment, diagnosis, and treatment for patients of all ages attending the Primary Care Centre for urgent care outside normal surgery hours. Seeing patients booked in via NHS 111, Clinical Assessment Service (CAS), Out-of Hours triage (OOH) and Emergency Department streaming, working through a base-clinic caseload at approximately 34 patients per hour. Conducting face-to-face consultations from the clinical base and managing a varied caseload including acute minor illnesses and exacerbations of chronic conditions. Assisting with telephone triage when there are lower levels of face-to-face consultations booked, providing expert clinical advice, risk assessment, and directing patients to appropriate care pathways. Utilising the Adastra clinical system to accurately record consultations and clinical notes. Dispensing medications from stock when required, in line with local protocols and safe medicines management. Working autonomously while collaborating closely with a multidisciplinary team - including doctors, nurses and administrative staff - to ensure seamless, patient-centred care. Making appropriate referrals to secondary care, liaising effectively with other services, and ensuring patient safety through sound clinical decision-making. Maintaining accurate clinical records and adhering to Practice Plus Group policies and NHS guidelines to uphold high standards of care.

About us - Registered Nurse with MSc in Advanced Clinical Practice or equivalent - Experience within Out of Hours, Primary Care GP Surgery, or Urgent Treatment Centre - Holder of V300 / Non-Medical Prescribing qualification and minimum 6 months experience post qualification. - Ability to autonomously see and treat patients of all ages - Confident clinical decision-maker with strong assessment skills. - Excellent communicator with a patient-centred approach. - Flexible, self-motivated, and committed to ongoing professional development.
