About this role

Job summary

The Bladder and Bowel Service is a multi-professional service working across BNSSG. Adults are seen in clinics to diagnose and treat continence or other bladder and bowel problems. A specialist domiciliary service is offered to housebound patients where the Integrated Network Teams requires additional expert guidance.

The Service is responsible for continence management within nursing & care homes and provide training and advice and guidance to a range of healthcare professionals. BABS also manage the contract for continence products on a home delivery system and collaborate with colleagues across BNSSG to work within the urology and colorectal formulary.

Current working hours of the service: Monday-Friday 8:30-16:30. This post is advertised as 18.75 hours which can be worked over 3 days (preferably over Wed-Fri).

BABS has a flexible hybrid working model. Staff are expected to work from an office base for part of their working week and travel across the BNSSG areas as required for patient appointments, meetings, training, service delivery, covering staff absences, and other operational service needs.

The post holder will work well within the team supporting care homes, health care professionals, patients and carers to provide comprehensive assessment, re-assessments, supporting training and advice and guidance.

The HCA will often work unsupervised, undertaking a range of activities and extended roles having received training and been assessed as competent

Main duties of the job

Undertake a range of delegated clinical and care led activities.

Undertake assessments / re-assessments and treatment plans for service users.

To provide a safe, patient centred, effective and evidence-based care.

Liaises with colleagues, patients/clients and/or external contacts regarding complex messages and patient queries including sensitive information.

Will be expected to respond appropriately to unexpected and challenging situations which may include contact with patients in emotional situations maintaining a calm and professional approach.

Work flexibly to meet the service needs and undertake any other duties that may be reasonably requested.

Monitoring long term conditions, care and treatment plans as per policy, protocol or guidelines, escalating when required.

Ability to work unsupervised.

To communicate effectively with patients and other stakeholders, verbally in writing and electronically using email. Includes writing clinical records in line with the Sirona Care & Health documentation policy.

Expected to follow standard operating procedures (SOPs) and policies to support your role and contribute to ideas for service improvements, e.g. data collection.

Prioritises own work to meet deadlines in conjunction with colleagues and prioritises on-going work with guidance.

Person Specification

Experience Essential

Significant experience of direct patient care in a health or social care setting Ability to plan and prioritise effectively and efficiently individually and in a team environment Experience monitoring long term conditions, care and treatment plans as per policy, protocol or guidelines, escalating to registered staff for support and guidance when required. Ability to work unsupervised. Able to record succinct and accurate information on the patient record both in writing and electronically

Desirable

Undertake assessments / re-assessments and treatment plans for service users as delegated by senior members of the team IT skills and use of record system such as EMIS (or similar) Experience in community and acute settings

Qualifications Essential

NVQ Level 3 in Health and Social Care (Adults) or equivalent experience
