Location: Princes Risborough Head Office & and travel to business locations in Buckinghamshire where needed

Hours: 37.5 hours per week covering 5 days

Salary: £32,073 per annum dependent on experience

Employment Type: Fixed Term until March 2029

Are you passionate about helping people overcome barriers to employment, improve their wellbeing, and achieve their personal goals? We are looking for a compassionate, motivated, and proactive Social Prescribing Link Worker & WorkWell Coach to join our multidisciplinary WorkWell team.

In this rewarding role, you will provide person-centred support to individuals experiencing health, wellbeing, social, and practical challenges that impact their ability to enter, remain in, return to, or progress within employment. Working closely with participants, healthcare professionals, employers, and community organisations, you will deliver holistic support that empowers individuals to improve their health, wellbeing, confidence, and independence.

Main duties of the job

You will manage a diverse caseload of individuals facing a range of challenges including physical and mental health conditions, disabilities, musculoskeletal issues, social isolation, housing and financial difficulties, caring responsibilities, and other factors affecting employment outcomes.

Using a social prescribing and coaching approach, you will undertake holistic assessments, develop personalised work and wellbeing plans, coordinate access to community and voluntary sector services, and support individuals to achieve sustainable employment and positive life outcomes.

About us

FedBucks is a not-for-profit GP federation of 45 GP practices covering a population of over 550,000 patients across Buckinghamshire. We began in 2016 and now employ around 300 members of staff at our head office site, and across our planned and unplanned care services.

Manage an active caseload of WorkWell participants. Contribute to delivery of WorkWell outcomes and key performance indicators. Work across community, primary care, workplace and virtual settings. Support integration of health, wellbeing, employment and community support services across the WorkWell pathway.Key Responsibilities

Social Prescribing and Personalised Care

Undertake holistic, person-centred assessments to identify health, wellbeing, social and employment-related needs. Develop personalised support plans based on individual goals, strengths and aspirations. Support individuals to access community groups, voluntary organisations and local support services. Address wider determinants of health including housing, debt, social isolation, transport, digital exclusion and caring responsibilities. Empower individuals to increase independence, resilience and self-management. Promote wellbeing, community connectedness and social inclusionWorkWell Coaching and Employment Support

Manage a caseload of participants experiencing health-related barriers to employment. Undertake holistic biopsychosocial assessments to identify barriers and opportunities relating to work and wellbeing. Support individuals to remain in work, return to work or move towards sustainable employment. Provide coaching, motivation and practical support to improve confidence, resilience and employability Support participants with CV development, job applications, interview preparation and skills progression Develop and review personalised work and wellbeing action plans Promote the positive relationship between good work, health and wellbeingCase Management and Care Coordination

Act as lead practitioner for allocated participants. Coordinate support across health, employment, skills, welfare and community services Facilitate warm handovers and supported referrals to partner organisations. Maintain regular contact with participants and monitor progress towards agreed outcomes. Identify and escalate safeguarding, welfare or clinical concerns in line with policy. Support participants through transitions such as returning to work, job loss, changes in health status or personal circumstances.Partnership and Community Engagement

Build and maintain relationships with community organisations, VCSE partners, local authorities, healthcare services and employment providers. Develop knowledge of local community assets and support pathways. Promote WorkWell and social prescribing services across partner organisations Work collaboratively with Occupational Health Advisers, Work and Health Coaches, Mental Health Practitioners and other MDT members. Participate in multidisciplinary meetings and case discussions.Employer Engagement

Support participants and employers to identify practical workplace solutions and reasonable adjustments Contribute to return-to-work planning and sickness absence prevention activities. Promote workplace wellbeing resources and support services. Signpost employers to specialist occupational health support where required.Performance, Quality and Governance

Maintain accurate and contemporaneous case records. Collect and record outcome data in accordance with service requirements Support service audits, evaluations and reporting requirements. Contribute to quality improvement and service development initiatives. Comply with safeguarding, information governance, health and safety and organisational policies.Essential Qualifications:

Educated to degree level or equivalent experience. Relevant qualification in Social Prescribing, Health and Wellbeing, Social Care, Community Development, Psychology, Public Health, Coaching or a related discipline. Evidence of continuing professional development. Understanding of personalised care and strengths-based practice.Desirable Qualifications:

Qualifications Essential

Educated to degree level or equivalent experience. Relevant qualification in Social Prescribing, Health and Wellbeing, Social Care, Community Development, Psychology, Public Health, Coaching or a related discipline. Evidence of continuing professional development. Understanding of personalised care and strengths-based practice.

Desirable

Accredited Social Prescribing Link Worker training. Health Coaching qualification. Level 3 or above Information, Advice and Guidance qualification. Mental Health First Aid qualification. Vocational Rehabilitation training. Motivational Interviewing training. Behaviour Change qualification.

Skills and knowledge Essential

Experience Essential

Experience supporting individuals with complex health, wellbeing and social needs. Experience of case management and personalised support planning. Experience working in social prescribing, community development, health, wellbeing or employment support services. Experience of partnership and multi-agency working. Experience supporting people experiencing mental health, physical health or social challenges. Experience maintaining accurate records and outcome reporting. Experience delivering coaching, motivational support or behaviour change interventions.
