Job summary
We are a team of Occupational Therapists and Physiotherapists working closely with the multidisciplinary team to prevent further deterioration in patients' physical functioning and wellbeing. We cover the areas of Chorley, Preston and South Ribble.
Main duties of the job
Your role will be to support the Occupational Therapists and Physiotherapists within intermediate care including undertaking non-complex assessments and carrying out treatment plans under the direction of the therapists.
We are looking for a flexible Assistant Practitioner/Technical Instructor, who is able to work across a range of settings and has experience working to support Occupational Therapy and Physiotherapy interventions within intermediate care.
The role involves working with patients in short term beds, at a dementia care unit located in Leyland and in patients' own homes. You will require the ability to travel to carry out home visits. We are looking for an individual with excellent communication skills and who has the ability to work autonomously as well as part of a team.

Education/Qualifications Essential
Assistant Practitioner Qualification or equivalent NVQ level 3 or equivalent Evidence of continued professional development
Knowledge Essential
Knowledge of long-term conditions Knowledge of mental illness and disability Knowledge of the cultures and religions of the area communities and to work across those cultures.
Experience Essential
Experience of rehabilitation in the community Demonstrates the ability to work with minimum supervision Ability to organise own time and workload
