Job summary
The post holder will work closely with the Operational leads and be a key member of the Referral to Treatment Team, proactively monitoring patients against the key performance indicator waiting times, in accordance with national and locally agreed Referral to Treatment pathways and targets.
Thepost holder will track against targets and raise alerts regarding potential breaches to appropriate senior team member of such breaches.
Main duties of the job
To monitor patient journey against the 18-week Referral to Treatment target ensuring that each patient pathway is monitored against agreed pathways and in accordance with the Trust's policy.Maintain robust data collection systems ensuring that clinical and non-clinical information, covering all aspects of the patient's 18-week journey, is collected in a timely fashion and entered onto the Careflow PAS.Monitoring and amending data input to always ensure accuracy.To liaise with services across the Trust regarding patient appointments with agreed timescales to ensure 18-week targets are met.To highlight potential breaches and identify appointments to be brought forward.Ensure all information is correctly recorded on Careflow PAS for all patients referred from another Hospital, to enable the Trust to monitor against all existing targets and patient pathways.To record any amendments to the patient journey on Careflow PAS.To participate in the weekly meetings and follow up any agreed actions with senior members of the team.To provide cover for colleagues on annual leave and sickness absence.To assist with the implementation of changes in working practices and modernisation etc, as discussed and agreed with the senior members of the RTT team.Ensure on-going, regular review and validation of Careflow PAS held information i.e. appointments, cancellations, DNAs, breaches, duplication and RTT records for all patients in the organisation.

Person Specification
Qualifications Essential
NVQ 3 or 2 A levels or equivalent experience
Experience/Skills Essential
Strong ability to analyse data, interpret complex rule sets, and apply strict attention to detail Excellent verbal and written communication skills to handle queries and collaborate with clinical governance teams Ability to prioritize a heavy, repetitive workload and to meet tight internal and external deadlines
Desirable
Previous experience using NHS Patient Administration Systems (PAS) and a background in RTT or medical administration are highly preferred.
