An exciting opportunity has arisen for an experienced Nurse Practitioner to join our clinical team at Huntcliff Surgery for 22.5 hours per week.
Based in the Victorian seaside town of Saltburn by the Sea, Huntcliff is a busy, forward thinking training practice caring for a growing population of approximately 10,000 patients.

Main duties of the job
Working as an autonomous practitioner, you will use advanced clinical assessment and decision making skills to assess, diagnose and manage patients presenting with a range of acute and chronic conditions, both within the practice and through domiciliary visits where clinically appropriate.
You will work collaboratively with our GPs and wider multidisciplinary team, independently prescribing within your scope of practice, interpreting investigations and referring appropriately where further specialist care is required.
The role will include health promotion, disease prevention and chronic disease management, alongside opportunities to contribute to clinical governance, audit, quality improvement and the review and development of clinical policies and pathways.

We have a list of approximately 10,000 patients and a multidisciplinary team of GPs, advanced and practice nursing staff, healthcare professionals and an experienced reception and administration team.

Key Clinical Responsibilities
1. To provide a first point of contact for patients with undifferentiated and undiagnosed health needs.
2. Provide domiciliary visits
3.Using advanced assessment skills of history taking and clinical examination, make a diagnosis and initiate an appropriate plan of care, including referral to other health and social care professionals as necessary.
4. Provide ongoing education and health promotion in relation to primary prevention/secondary prevention.
5. Organise and manage own patient caseload.
6. Request, interpret and evaluate diagnostic tests to assess patients' clinical problems or monitor progress.
7. Manage patients with long term conditions, including the development, implementation and evaluation of individualised treatment plans.
8. To provide a holistic nursing assessment drawing on knowledge about the wider determinants of health and ill health.
9. Independently prescribe medication within own scope of practice and undertake medication reviews in accordance with current guidance.
10. Participate in the development and implementation of Practice/PCN and wider system clinical pathways and guidelines where appropriate.
11. Provide clinical care within own scope of practice and recognise, assess and appropriately refer patients presenting with mental health or safeguarding needs.
12. Provide expert nursing advice to other nurses working in practice/primary care setting.
13. Establish links with Secondary Care providers to help deliver changes in care pathways and protocols.
14. Work with nursing/medical teams in providing proactive management of patients with chronic diseases in accordance with local/national policy.
15. Collating data and involvement in Chronic Disease Management for disease specific areas including Hypertension, Diabetes, Cardiovascular Disease, Asthma and COPD as part of the Quality and Outcomes Framework.
16. Reviewing patients on other registers including Mental Health, Frailty and Epilepsy Registers and inputting data where appropriate as part of the Quality and Outcomes Framework.
17. Undertaking audit of patients requiring review of further intervention.
Clinical Developments
1. Establish initiatives which will contribute to improving access to quality services such as rapid access to minor injuries/ minor illness, Chronic Disease management.
2. Contribute to the establishment and maintenance of peer clinical supervision and appraisal.
3. Take an active role in clinical governance, quality improvement, clinical audit and significant event/ near miss review, contributing to the development and implementation of clinical policies, pathways and guidelines.
General Responsibilities.
1. To contribute to the overall development and implementation of clinical developments at practice level.
2. Provide evidence of continued professional development.
3. Participate in regular and ongoing appraisal, taking responsibility for professional competency.
4. To be aware of and utilise risk management strategies both clinical and organisational in all aspects of practice.
5. Maintain current NMC registration and practise in accordance with the NMC Code and revalidation requirements.
6. To comply with health, safety and welfare of self and others within the requirements of health and safety regulations.
7. To ensure confidentiality at all times and comply with the practice Caldicott requirements.
Person Specification
Experience Essential
* Experience of working in primary care. * Experience of assessing and managing acute and chronic conditions. * Experience of prescribing and undertaking medication reviews. * Experience of chronic disease management. * Evidence of safe clinical decision making and working within own scope of practice. *Working knowledge of the British National Formulary.
Desirable
* Experience of clinical audit and quality improvement. * Experience of clinical risk management. * Triage experience * Relevant chronic disease qualifications/competencies. * Women's health

* Access to appropriate transport and ability to travel within the locality to undertake domiciliary visits
Qualifications Essential
Relevant Nursing Qualification (RGN or equivalent) An Independent Prescriber Qualification Nurse Practitioner Qualification.
