Social Worker (LMSW Required) - Hospice Serving Dillon & Horry Counties, SC Make a Difference When It Matters Most We are seeking a compassionate and dedicated Licensed Master's Social Worker (LMSW) to join our Hospice team. In this rewarding role, you will provide emotional support, counseling, advocacy, and resource coordination for patients and families navigating end-of-life care. Working collaboratively with an interdisciplinary team, you will help ensure patients receive the support and resources needed to enhance quality of life and dignity throughout their healthcare journey. Key Responsibilities
• Assess patient and family psychosocial needs and develop appropriate care interventions. • Provide counseling, emotional support, and crisis intervention when needed. • Advocate for patients and families while helping them navigate complex healthcare and community systems. • Collaborate with physicians, nurses, case managers, and other healthcare professionals to support individualized care plans. • Coordinate referrals and connect patients with community resources and support services. • Complete required assessments, level-of-care documentation, and other regulatory requirements. • Assist vulnerable populations by coordinating resources through agencies such as APS, CPS, DSS, and disability support organizations. • Address patient and family concerns promptly and professionally while ensuring exceptional customer service. • Participate in on-call social work consultation as needed. • Full-Time • 72 Hours Bi-Weekly • Primary coverage area includes Dillon and Horry Counties • Based out of Dillon, SC Qualifications
• 1-3 years of related social work experience preferred. • Strong communication, counseling, documentation, and organizational skills. • Computer proficiency required. • Knowledge of social work principles, practices, and community resources. • Must possess a valid South Carolina driver's license and reliable transportation. • Ability to provide social work consultation support on a 24-hour basis as needed. Education & Licensure Requirements
• Master's Degree in Social Work from a program accredited by the Council on Social Work Education (CSWE). • Current Licensed Master's Social Worker (LMSW) license in South Carolina. • Minimum of one year of experience in a healthcare setting preferred. Why Join Us?
