Description: "Are you a relationship-driven registered dietitian with a passion for enteral nutrition, patient education, and helping patients transition safely from hospital to home? Join IU Health Home Care as a Dietitian Referral Liaison in Fort Wayne—a unique opportunity to combine your clinical expertise, relationship-building skills, and interest in referral growth. In this embedded, field-based role, you’ll collaborate with hospital teams, physicians, dietitians, case managers, patients, and caregivers; coordinate home enteral nutrition referrals; provide tube-feeding and pump education; troubleshoot clinical needs; and build strong referral relationships throughout the Fort Wayne area. This is an ideal role for a motivated dietitian who enjoys variety, independence, travel, and making an impact beyond traditional clinical practice." This is a full-time, in-person Clinical Dietitian Liaison role focused on helping pediatric and adult patients successfully transition home on tube feedings. Knowledge of or experience with enteral nutrition, tube-feeding regimens, formula recommendations, and patient education is preferred. This unique dietitian role combines clinical expertise, patient and caregiver education, referral coordination, relationship development, and business growth. The position is Monday-Friday and requires regular travel throughout the Fort Wayne area. The liaison will provide coverage to designated hospitals, including:

• The new IU Health hospital planned for the Fort Wayne area • Parkview Hospitals • Lutheran Hospital • Dupont Hospital • Other surrounding hospitals, physician offices, and clinics as assigned The eight-hour workday may generally be scheduled between 8:00 a.m. and 6:00 p.m., with flexibility based on referral volume, hospital needs, discharge timing, and patient education needs. The successful candidate should live within the Fort Wayne service area and be able to travel regularly among assigned facilities. Position Responsibilities Promotes IU Health Enteral Nutrition services to drive referral growth and retention from IU Health and non-IU Health providers, serving as a liaison to deliver tailored education, build trusted relationships, and support market strategy development. Coordinates with internal teams to ensure complete patient documentation. Ensures timely and safe transition of care to home by monitoring patient status, providing pump education, addressing clinical and insurance concerns and through collaboration with referral sources and prescribers. Successful engagement of referral sources is accomplished by developing and maintaining relationships, demonstrating expertise, delivering on promises, and by becoming a trusted resource. Essential Job Functions

• Check on status of new and existing patients, provide instructions on pumps, ensure patient and/or caregiver knows what to expect while on service • Complete documentation for new patient referrals; Work with Patient Care team to ensure we have all appropriate paperwork to set up a patient on service • Respond to patient questions and concerns; assess patient needs, status, and tolerance issues, troubleshoot and make recommendations as indicated or requested • Interface with physicians, dietitians, and case managers to clarify and manage patients’ home feeding regimen • Respond and follow up with Revenue and Patient Care requests, including insurance issues, medical eligibility, and clinical information for authorizations • Identify growth opportunities • Cold call hospitals/offices to create new leads • Build relationships with key players within the hospitals to grow referrals • Schedule presentations, in-services, meetings to discuss enteral services • Collaborate with other enteral RD liaisons and marketing team to create appropriate and personalized collateral needed to enhance marketing efforts • Assist with coverage of other liaisons whenever necessary. Requirements

• Bachelor's Degree is required. • Requires 3-5 years of relevant experience. • Registered Dietitian required. Effective January 1, 2021 all dietitians must have an active Indiana license. • Requires strong interpersonal and communication skills. • Knowledge of health services and clinical needs of enteral nutrition population preferred. • Experience collaborating with multidisciplinary teams, including but not limited to case managers, clinical staff, and healthcare providers preferred. • Requires the ability to travel in service area on a regular basis. • Sales experienced preferred.
