You will play a key role in providing and maintaining a high standard of care to our patient population, working closely with GPs, nurses, and the wider multidisciplinary team. Our patient demographic is mixed but weighted to working age patients leading to an interesting chronic disease workload and screening activity. We have a highly organised and skilled administration team who effectively signpost patients to other services and have an embedded workflow management system to reduce the administrative burden.

Main duties of the job

In this role you will work independently, making professional, autonomous decisions.

The role will involve initial history taking, clinical assessment, diagnosis, treatment plan and evaluation of patient care.

You will be responsible for making necessary referrals, requests for investigations.

Involve independent prescribing with the scope of professional practice.

Job description Job responsibilities

Our ANP will be responsible for delivering high-quality health services across the practice, including but not limited to the following

The post holder should be able to see all patients that present including children, adults and those experiencing mental health challenges.

Develop, implement and embed health promotion and wellbeing programmes

Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

Implement and evaluate individual specialised treatment plans for chronic disease patients

Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

Provide advanced, specialist nursing care to patients are required in accordance with clinical based evidence, NICE and NSF

Undertake the collection of pathological specimens

Provide travel medicine services

Request pathology services as necessary

Process and interpret pathology and other test results as required

Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

Maintain accurate clinical records in conjunction with extant legislation

Ensure SNOMED CT codes are used effectively

Maintain chronic disease registers

Prioritise health issues and intervene where necessary

Support the team in dealing with clinical emergencies

Recognise, assess and refer patients presenting with mental health needs

Be an extended and supplementary prescriber, adhering to extant guidance

Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

Contribute to practice targets (QOF, LQC etc), complying with both local and regional guidance

Liaise with the external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc)

Delegate clinical responsibilities where appropriate (ensuring safe practice and that the task is within the scope of practice of the individual)

Support the clinical team with all safeguarding matters in accordance with local and national policies

Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

Deliver opportunistic health promotion where appropriate

Perform audit of aspects of practice nursing work.

Person Specification

Qualifications Essential

Registered General Nurse ( currently registered with the NMC) Recognised ANP qualification at minimum of Master Level or equivalent Qualified Independent Nurse Prescriber on the NMC register Has evidence of working at an enhanced level, as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework Meets NMC revalidation requirements in accordance with the NMC Confirmation of registration with the NMC

Desirable

Experience of working in a GP practice.

Understanding of general practice and the wider NHS Experience of leading in chronic disease management Experience of prescribing and undertaking medication reviews Experience of infection prevention and control measures
