Job summary
We are a community service providing care and support for patients in their own homes preventing hospital admission and facilitating early hospital discharge.
We work closely with primary and secondary care taking referrals from both hospitals and GPs.
We have a good rapport with our patients and get excellent feedback so job satisfaction is high. We take pride in enabling patients to stay at home when acutely unwell.
Our main priority for this job role is to find people with the right values, behaviours and attitudes that match our own. Previous experience is an advantage but we can provide training if necessary.

Main duties of the job
Our aim is providing treatment and care for people in their own homes.
As a band 3 you would be a support for the band 5 Community Staff Nurses visiting patients to monitor observations, obtain venous blood samples, provide wound care, education and support, this list is not exhaustive.
As a Community Support Worker you will be lone working visiting patients home within the locality, you must have access to a vehicle which can be used for work purposes or are you able to travel as required to meet the needs of the service.

Education & Qualifications Essential
NVQ/Diploma Level 3 in Care CPD Diary effectively demonstrating levels of learning and reflection from courses / experiences
Skills, Knowledge & Abilities Essential
Knowledge of Health Promotion Good communication skills.
Experience Essential
Awareness of Health and Safety and Manual Handling requirements Experience of working in a Health/Social Care/ related environment
