Job summary
We are looking for an enthusiastic and passionate registered nurse to join the Southampton Tissue Viability Team.
Our mission is to provide an open and honest, holistic, collaborative, specialist tissue viability service that facilitates the highest quality, evidence-based care, which keeps our patients at the heart of everything we do.
The team is based at the Adelaide Health Centre and will see patients in the Community, Community Hospitals, GP Practice, Nursing Homes and in Clinic.
Our role is to support health care professionals in clinical practice by providing teaching, information and specialist advice on tissue viability.
We provide:
Complex wound assessment and advice
Leg ulcer service, including lower limb assessment
Telephone advice
Training and education
Supervision and competency review
Link nurse meeting and training
The successful candidate will support patients and their families to achieve improved health outcomes and promote health and wellbeing, while demonstrating care, compassion, competence, communication, courage, and commitment.
Care is based on comprehensive assessments and is patient/client centred, culturally sensitive, evidence-based and promotes independence.
The role will develop and maintain a culture of service improvement which promotes quality, safety and striving to do better.
Main duties of the job
Provide specialist Tissue Viability service across clinic, home and community settings, delivering assessment, diagnosis, treatment, care planning for patients with wounds, ulcers and compromised tissue viability.
Promote health and preventative care through risk assessment, education and evidence-based interventions to reduce wound breakdown, recurrence and health inequalities.
Safeguard children and adults, obtaining and documenting informed consent for care and information sharing in line with Trust policies and professional standards.
Deliver high-quality, person-centred nursing care through ongoing assessment, planning, implementation and evaluation, maintaining accurate clinical records.
Manage a defined Tissue Viability and Leg Ulcer caseload, ensuring timely admission, discharge and onward referral, contributing to effective use of resources.
Assess and manage complex wounds, including wound healing evaluation, pressure ulcer classification, lower limb assessment, Doppler assessment and appropriate use of wound care formulary products, escalating care when required.
Communicate sensitively with patients and carers regarding treatment plans and changing conditions, providing support to promote physical and psychological wellbeing.
Demonstrate clinical autonomy, promote best practice, support service improvement and positive patient outcomes, role model culturally safe care, challenge poor practice and contribute to the development of knowledge and skills within the wider team.

Job description Job responsibilities
1. Clinical and Professional
1.1 To provide a Tissue Viability service in a clinic setting, as well as the patient's own home and other community settings, which includes:
Tissue Viability assessment, diagnosis, care, treatment, and referral.Preventative health education/promotion and care: Risk awareness (assessment), proactive care and management to minimise or prevent wound or ulcer breakdown or recurrence.Public health: health protection and promotion programmes that improve and reduce inequalities.To actively safeguard and promote the welfare of children and adults.Obtain informed consent from patients for assessment and intervention, and work within agreed guidelines for patients unable to give informed consent.Obtain consent to the sharing of information, and document all consent issues in accordance with relevant Trust procedure.1.2 To take responsibility for providing the highest standard of patient care through on-going assessment, planning, implementation and evaluation of care.
1.3 To co-ordinate, organise and manage a defined caseload within the Tissue Viability/Leg Ulcer Service, and enabling the patients' needs to be met within the limits of available staffing resources including supervision and support for junior staff.
1.4 To be competent in identifying patients for admission to the caseload, and discharges and/or onward referral as appropriate following a complete episode of nursing care.
1.5 To participate in regular multi-disciplinary reviews of patients with chronic/palliative wounds, ensuring that holistic assessments are carried out and care plans are evaluated to ensure they are up to date and relevant.
1.6 To be competent in performing a Holistic Nursing Assessment including recording and interpretation of vital signs.
1.7 To be competent in managing patients with compromised Tissue Viability including appropriate use of wound care formulary, evaluation of wound healing, recognition and categorisation of pressure ulcers or reduced perfusion to lower limbs, performing Doppler assessment and referral for specialist opinion, as required.
1.8 To deal empathetically with sensitive information, to include discussing patient's changing condition with patient and / or carer.
1.9 Accurately documents assessments of patients/client's health status, diagnosis and response to nursing and other interventions and treatments on the Trust Clinical Records System.
1.10 To have responsibility, authority and clinical autonomy of a link nurse role to develop and improve care, building on existing good practice and clinical skills development.
1.11 Fosters inquiry and critical thinking amongst colleagues to advance nursing practice and patient/client care to ensure the provision of positive patient/client outcomes and person-centred care.
1.12 Support a health promotion in supporting, guiding and instructing patients and carers to maintain their optimum level of physical and psychological health.
1.13 Role models culturally safe nursing practice and able to report poor practice when observed.
1.14 Safely manage high risk areas, including:
Lone working in the community.Patient handling in challenging environment where the principles of best practice regarding patient handling may be compromised.Challenging behaviour situations.Handling medical sharps and adherence to infection control policy.Handling body fluids and other hazardous material.2. Communication
2.1 Provides and receives complex, sensitive information to patients/relatives and colleagues and with empathy, persuasion and reassurance and ensure information is clearly understood, where there maybe barriers to understanding.
2.2 Ensures effective collaboration and communication with multidisciplinary team, patients, carers, family and significant others, always seeking consent of the client in doing so co-ordinating care and fostering effective working relationships to achieve best health outcomes.
2.3 Actively listens, drawing out information and checks understanding.
2.4 Empathises with others and considers their needs and feelings.
2.5 Expresses information effectively and accurately, both orally and in writing, adjusting language and style to recipients and considers their frame of reference.
2.6 Accurately records patient information, progress and current status ensuring changes in patient/clients' condition are shared with the multi-disciplinary team in a timely manner.
2.7 Ensures patient confidentiality is always maintained.
2.8 Participate in staff meetings, case conferences and other meetings/working parties as required.
2.9 Promotes Hampshire & IOW NHS Foundation Trust as a centre of excellence for nursing practice through effective communication both internally and externally.
3. Analytical / Judgement skills
3.1 Be personally accountable for actions and omissions in your practice and have the ability to justify decisions taken.

4.1 Provide day to day clinical supervision of junior colleagues and students including safeguarding supervision when appropriate.

4.3 Actively participate in clinical and management supervision.
4.4 Works in accordance with the relevant Health and Safety Trust policies, procedures, and guidelines.
4.5 Attends organisational mandatory training as required by the Trust and relevant to your role.
4.6 Actively participates in and enables others to carry out risk assessments within the working environment.
4.7 Ensures self and colleagues are trained and competent to use equipment safely.
4.8 Ensures incident reporting is expedited promptly and appropriate action plans completed.
5. Policy and Service development
5.1 Maintains and develops own clinical expertise and knowledge in speciality evidence-based nursing practice and personal portfolio to demonstrate fitness to practice in line with expectations for revalidation.
5.2 Identifies situations of clinical risk and takes appropriate action to ensure a safe environment for patients/clients/families and staff.
5.3 Contributes to evidence-based nursing practice in speciality areas.
5.4 Demonstrates commitment to quality improvements, risk management and resource utilisation participating in monitoring and evaluation activities including audit and research activities.
5.5 Provides input into clinical standards/protocols and policies and undertakes clinical audits as required.
5.6 Evaluates the effectiveness, efficiency and safety of clinical practice contributing to service improvement initiatives.
5.7 Participates in the implementation of nursing models of care appropriate to patient/client population needs.
5.8 Contributes to and participates in Hampshire & IOW NHS Trust policy development providing own expertise where relevant and feedback on proposed polices based on relevant experience and expertise where applicable.
5.9 Promote patient and public involvement in activities designed to inform service improvement.
6. Research and development
6.1 Pro-actively participates in own performance development and review to identify learning needs and take necessary steps in order to reach personal/professional objectives.
6.2 Attends educational opportunities relevant to nursing role and scope of practice.
6.3 Actively participate in the induction of new staff in accordance with Trust policy and when required act as a 'buddy', assessor/supervisor and resource person within your team.
6.4 Participates in peer review/feedback and actively contributes to the provision of an effective learning environment.
6.5 Provide opportunities for junior colleagues to assess their work and to provide them with clear, sensitive, and appropriate feedback that will help them improve and develop.
