A vacancy has arisen for an Assistant Practitioner in the Walsall ESD/Community Stroke Team.

We are looking for an enthusiastic, motivated individual with a passion for stroke, in stroke to join our excellent established interdisciplinary team. The team consists of Physiotherapists, Occupational Therapists, Speech and Language Therapist, Specialist Nurses, clerical officer and Assistant Practitioners. The team is based in Darlaston, and works primarily in a community setting in the Walsall and surrounding areas.

Main duties of the job

The successful candidate will assist in the rehabilitation of patients post stroke, in their own homes. With guidance from qualified staff members, they will facilitate client participation in their treatment / rehabilitation programmes for all disciplines. They will support the team with data collection and audits to continually improve the service. They autonomous practitioners and will be responsible for organising, prioritising and planning their own workload to meet client and service priorities.

Qualifications Essential

5 GCSE's or equivalent Level 3 apprenticeship or equivalent L5 qualification

Experience Essential

Experience in a NHS health care environment working as part of a multi-disciplinary team. Experience of working with stroke patients Experience of working in a community health care setting Experience of working in a rehabilitation setting, delivering Physiotherapy and/or Occupational Therapy care plans and/or SLT Able to assist registered staff in assessing a person's, delivery of therapy and nursing care, monitoring and reviewing the person's condition. Able to work independently within limits of skill and knowledge and aware of when to seek advice. Must be able to accurately follow instruction from a supervising health professional and to work as a team with all members of the department and Multidisciplinary Team Strong communication skills. Demonstrable basic physical assessment skills Basic case management skills Good organisational skills. Competent in IT/word processing skills. Time management skills. Analytical/ problem solving skills. Ability to work under pressure.

Desirable

Ability to manage/prioritise workload autonomously.

Skills and Aptitudes Essential

Demonstrates effective IT skills, including the ability to operate Microsoft word and e - mail and other applications Demonstrates the ability to work under pressure Demonstrates the ability to work within an evolving team and adapt to change.

Key Competencies Essential

Able to demonstrate empathy, motivate and encourage service users with impaired cognitive abilities or physical senses to participate in the treatment process Able to work flexibly and respond to changing demands and priorities. Demonstrates the ability to travel frequently throughout the borough and surrounding locations during the working day.

Flexibility Essential

Must be willing to work across the 7-day week as the service requires Must be willing to work and support any of the teams within Walsall Integrated Stroke Service (Walsall, Cannock & Stafford Community Stroke Teams) Demonstrates the ability to travel frequently throughout the borough and surrounding locations during the working day. Demonstrates the ability to work in MDT which include both clinical & non -clinical personnel
