Reporting to the Director of Wellness, the Health Programs and Services Manager oversees the development, implementation and delivery of home and community care, Elders support, community health, patient navigation and health-related programs and services. In addition to coordinating client case management and supporting Citizens experiencing complex health, mobility, aging, accessibility, chronic disease and acute health challenges, the incumbent leads complex and high-risk health cases and case planning, while ensuring that appropriate systems, procedures and service delivery frameworks are in place to support effective and coordinated care. The incumbent contributes to program development, funding initiatives, budget administration and community engagement activities, and liaises with governments, agencies and community partners to identify and respond to emerging health and wellness needs. The incumbent supervises and supports a team of jëje’in and ensures programs and services are delivered in accordance with TH values, legislation, policies and strategic priorities.
An Eligibility List may be established from this competition – the duration of the eligibility list may be up to 12 months. The eligibility list may be used to fill future permanent full-time, permanent part-time, temporary full-time, temporary part-time, term, or auxiliary on-call vacancies within the same department and classification based on the organizational needs by going the next highest ranked candidate until the eligibility list expires.

Essential Qualifications:
• Post-secondary degree in nursing, health administration, public health, social work, human services or a related field of study, combined with five years of experience in managing community-based health, wellness or human services programs; an equivalent combination of education, knowledge, skills and experience may be considered.
• In-depth knowledge and understanding of home and community care, chronic disease management, care coordination, aging-in-place approaches, health system navigation across the continuum of care, and factors influencing individual, family and community health and wellbeing.
• Experience in working with First Nation governments and collaborating with health care providers, governments, non-government agencies and multidisciplinary teams in the delivery of community-based health and wellness services.
• Knowledge of community, territorial, federal, NIHB and other Indigenous health programs, services and resources available for planning, referral and coordination purposes.
• Knowledge and understanding of the importance of land-based healing initiatives in advancing First Nations community wellness.
• Demonstrated experience with budget and financial management.
• Demonstrated experience with managing, supervising, evaluating and coaching jëje’in.
• Knowledge and understanding of trauma-informed, culturally safe and person-centered service delivery approaches.
Assets:
• Experience in conflict resolution, mediation, negotiation and problem solving.
• Knowledge of restorative justice approaches.
• Knowledge of TH Government structures, culture, traditions, history, demographics, family structures, socio-economic dynamics, challenges and aspirations.
• Knowledge of the TH Final Agreement, TH Self-Government Agreement and the TH Constitution is an asset.
Conditions of Employment:
• Criminal record check with vulnerable sector screening.
• Class 5 driver’s license with a clear driver’s abstract, and willingness to acquire Class 4 driver’s license .
• Standard First Aid Certification
• TH101 Cultural Awareness training.
• Mental Wellness First Aid, Vicarious Trauma and Trauma-Informed Care Certification
