Rosedean House Surgery is looking for an experienced Advanced Clinical Practitioner to join our multi-disciplinary team, to help us to transform and modernise pathways of care, enabling the safe and effective sharing of skills across traditional professional boundaries.

Main duties of the job

The Advanced Clinical Practitioner will make autonomous decisions in the assessment, diagnosis and treatment of patients registered with our practices. You will have experience within a primary care setting and your high standards of patient care will naturally inspire those you work with. In addition to standard nursing duties, the advanced clinical practitioner (ACP) role is accountable in conjunction with the lead nurse for the clinical governance of the practice and is designed to provide clinical leadership in the practice, in order to ensure the safe, effective and compliant delivery of services to all patients.

You'll be working with a great local team and will have the support of the wider NHS services leadership team and our support centre teams as well. You will work alongside a multi-disciplinary team including: GPs, practices nurses, wellbeing advisors, health coaches, physician associate, and more.

The successful candidate will demonstrate flexibility in attitude and behaviour. They will be keen to undertake new ways of working and delivering healthcare, committing to undertaking the professional development required to adapt to changing circumstances.

What youll work on

Delivering safe, effective, and compliant care to patients across the practice Making autonomous clinical decisions in assessment, diagnosis, and treatment Managing planned and urgent primary care presentations, including minor illness Working closely within a multidisciplinary team to ensure patients receive the right care at the right time Contributing to clinical governance and quality improvement, alongside the Lead Nurse Providing clinical leadership within the practice and supporting high standards of care

Person Specification

Qualifications Essential

Registered with NMC / HCPC Have a MSc in Advanced Practice or Relevant ACP courses at Level 7 including recognised modules in Advanced Physical Assessment Skills (essential), Minor Injuries, Disease Management, etc. Certified Independent Prescriber (V300)

Experience Essential

Prior experience as an ACP in primary care

Knowledge, Skills and Abilities Essential

Confident in managing whole episodes of care while working collaboratively within a wider multidisciplinary team Demonstrate strong clinical reasoning and sound decision-making Have excellent communication skills, patience, and a friendly, approachable manner Able to perform effectively in complex and high-pressure situations Flexible, proactive, and able to inspire and lead others
