• Complete forms attached to file most especially the insurance approval form prior to consultation.

EDUCATION: Bachelor’s Degree/diploma or Equivalent EXPERIENCE: Minimum 2 - 3 years’ experience; OR any equivalent combination of experience and /or education from which comparable knowledge, skills and abilities have been attached
