Main duties of the job
As a care coordinator, you will be involved in proactive, person-centred care planning with people, overseeing and coordinating their care when needed, by bringing together the different specialists whose help the individual may need.
From the individuals perspective and their carers, nobody is potentially more highly valued than the care coordinator - you will be their key point of contact.

This post will be PCN Grade 4, subject to EMIS experience (essential), reporting to the PCN Operations Manager. CC experience desirable (from a healthcare setting not care home). We offer training for GP registrars, FY2 doctors, trainee practice nurses, pre-reg pharmacists and undergraduate medical students. You will work with a group of highly motivated individuals, passionate about using technology to improve services for patients. We work as a team, supporting each other in a friendly atmosphere. Our spacious staff rooms allows discussions to take place away from the workplace.
Excellent communications skills with patients, staff & seniors and in particular within the CC team is an essential quality which will be tested at interview.
Job description Job responsibilities
You will be self motivated and preferably have experience in general practice or a health care setting.
Main duties will be:
To support holistic care planning and co-ordination of inputs from a range of different professionals in an agreed care plan.2. To provide access to services and information for individuals and their carers.
3. To understand and coordinate the range of specialist clinical and support teams who can provide assistance.
4. To facilitate regular reviews of the personalised care plan.
5. To provide accurate and timely performance reporting.
6. To work within all policies and procedures ensuring information remains confidential.
7. To work with the relevant leadership team to develop and evaluate the role and to undertake other duties that are consistent with the duties and responsibilities of this post.
8. The proactive recall of patients for QOF and non-QOF targets. The care coordinator will contact diabetic patients, parents for child immunisations, patients for pandemic vaccinations, women eligible for smears, CHD patients for BP checks and patients for bowel screen when due.
9. The care coordinator will attend meetings for planning improvement on performance targets e.g. LTCs, screening, immunisation, NHS health checks, LD checks and a range of other service related targets.
10. The care coordinator will work closely with the practice manager and clinical service leads to support the setting up of clinics. They will help plan forward capacity and monitor effective appointment utilisation by patients. This will include the administration of clinical rotas and appointment sessions on the clinical system, allowing for annual leave, study leave and other absence.
11. The care coordinator will act as 'session lead' for mass vaccination clinics, coordinating the rota, supporting internal and external clinicians to start up IT systems and troubleshoot e.g. passwords. Coordinate vaccine allocation for the session and to individual clinicians. Minimise vaccine wastage and ensure that the cold chain is maintained at all times. Top up clinical supplies and ensure a tidy clinic. Safely ensure that clinical waste e.g. sharps is properly disposed. Monitor clinic throughput and escalate effectively to manage queues. Respond to patient queries on eligibility and book appointments. Support volunteers e.g. queue marshals with patient queries and feedback about the service. Report vaccine usage / wastage and update fridge stock sheets.

Qualifications Essential
GCSE Maths and English (C grade or above) or equivalent Evidence of continued professional development Good level of spoken English in tactfully liaising with patients, who may be experiencing hesitancy with health services and multiple appointments or be anxious generally (this will be tested at interview)

Diploma Level 2 in Health and Social Care or equivalent qualification
