West View Surgery is seeking an enthusiastic and motivated Primary Care Nurse / Practice Nurse to join our friendly, supportive and well established GP practice.

Main duties of the job
The post holder will be responsible for delivering safe, effective nursing care within a general practice setting. Duties will include, but are not limited to:
Management of longterm conditions (e.g. asthma, COPD, diabetes, hypertension) Immunisations and vaccinations Cervical screening Wound care, dressings and removal of sutures Health promotion and lifestyle advice Supporting QOF, enhanced services and chronic disease reviews Maintaining accurate and timely clinical records Working collaboratively with GPs, HCAs, pharmacists and the wider practice team Adhering to practice policies, clinical governance and infection control procedures

We are a friendly, forward thinking training practice in a pleasant market town set between Bristol and Bath (5 miles either way). West View Surgery has around 8,000 patients and we have two excellent purpose-built premises in Keynsham. Our three GP partners seek innovation in primary healthcare and are open to new ideas and ways of working. CQC rated the practice as good. We have full nursing and administrative support, employ via the PCN pharmacists who deal with most medication queries, First Contact Physios and a Paramedic and have the support of an Home Visiting Service. Job PurposeTo provide high-quality, evidence-based nursing care to patients within a general practice setting, working as part of a multidisciplinary primary care team. The post holder will contribute to the delivery of long-term condition management, preventative care, health promotion and enhanced services, ensuring safe and effective patient care at all times.
Main Duties and Responsibilities Clinical Responsibilities Deliver comprehensive nursing care within a primary care setting
Undertake long-term condition management including asthma, COPD, diabetes and hypertension
Administer immunisations and vaccinations in line with national and local guidance
Perform cervical screening in accordance with NHS standards
Provide wound care, dressings and removal of sutures
Deliver health promotion, lifestyle advice and patient education
Support QOF, enhanced services and local initiatives
Professional Responsibilities Maintain accurate clinical records using the practice clinical system
Work in accordance with the NMC Code of Professional Conduct
Adhere to practice policies and clinical governance frameworks
Participate in audit, training and service improvement activities

Person Specification Essential
Registered Nurse (NMC) Experience of working autonomously and within a clinical team Excellent communication and interpersonal skills Ability to manage workload effectively and prioritise care Commitment to high standards of patient care and confidentiality Experience working in primary care or general practice Chronic disease management qualifications (e.g. asthma, COPD, diabetes) Experience of cervical screening and immunisations Knowledge of QOF and enhanced services Experience using GP clinical systems (e.g. Systemone)
