Job Purpose

To provide care coordination and administrative support to patients, helping them access appropriate services and ensuring effective communication between patients, the practice and wider healthcare services.

Assist with general administrative duties, including filing, scanning, referrals and processing documents. Support the practice with PCN and practice projects and initiatives. Maintain confidentiality and comply with practice policies, GDPR and information governance requirements. Work as part of the wider practice and PCN team to improve patient access and experience. Undertake any other reasonable duties required by the Practice Manager.Person Specification

Good communication and interpersonal skills. Organised and able to manage workload effectively. Good IT and administrative skills. Ability to work independently and as part of a team. Professional, friendly and approachable manner. Understanding of confidentiality and handling sensitive patient information. Willingness to support reception, telephone and administrative duties as part of the role.Desirable:

Previous experience within a GP practice, NHS or healthcare setting. Experience of care coordination or working with patients requiring additional support. Knowledge of EMIS or other primary care systems. Understanding of PCN services and local community services.
