About this role

Job summary

We are looking for an energetic, motivated and enthusiastic Practice Nurse to join our friendly and supportive nursing team which consists of an ANP, 3 Nurses and 3 HCAs.

The post-holder will be proficient in all aspects of general practice nurse duties, and able to deliver high-quality, evidence-based nursing care to our patients, working autonomously within their scope of practice.

You will play a key role in managing long term conditions, delivering immunisations, supporting public health programmes, and promoting health and wellbeing across our local community. This is an exciting opportunity to contribute to improving patient outcomes, reducing health inequalities, and supporting service development within a forward-thinking, busy GP teaching practice.

Main duties of the job

Deliver routine nursing care including wound care, ECGs, and health checks

Lead and support the management of long-term conditions e.g. diabetes, COPD, asthma

Deliver immunisations, vaccinations and cervical screening programmes

Provide health promotion and prevention advice to patients

Support infection prevention and control IPC standards

Maintain accurate clinical records using appropriate coding systems

Participate in clinical audits, quality improvement, and service development

Work collaboratively within the multidisciplinary team MDT and wider PCN

Recognise clinical boundaries and refer appropriately to GPs or other professionals

Support the delivery of QOF targets and enhanced services

Person Specification

Qualifications Essential

Registered Nurse with the Nursing and Midwifery Council NMC Competence in core nursing skills e.g. immunisations, wound care, cervical screening Ability to work independently and within a team Strong communication and interpersonal skills

Desirable

Experience working as a Practice Nurse Experience in management of long term conditions Experience in quality improvement or audits Knowledge of QOF and NHS primary care systems Understanding of safeguarding, clinical governance, and evidence-based practice IT proficiency and ability to maintain accurate clinical records
