Job summary
Social prescribers support patients with various physical and mental health conditions by connecting them into community services, groups, activities and other pursuits.
They:
Promote prevention & lifestyle changesEngage with patients and connect them to a wide range of community groups and services, activities and other pursuitsTake a holistic approach for patients with chronic diseasesSupport patients with common mood disorders to access available servicesBereavement support, providing practical guidance and signposting them to an appropriate bereavement services where necessaryPromote and raise awareness of the social prescribing service within the practices and community organisations
Main duties of the job
Our SPLWs have a variety of duties, that include:
Working with patients to coproduce a simple and tailored action plan suited to their priorities, interests and motivations and designed to recover their health and overcome any barriers to success.
Maintaining excellent community knowledge and undertaking research to identify the best services, groups, activities and other pursuits for a person's specific needs and goals.
Encouraging people to take independent steps to improve their wellbeing.
Facilitating new and innovative solutions to complex situations as well as common barriers faced by people using the service.
Working as part of the PCN multi-disciplinary teams across allocated PCNs taking part in meetings, promoting the service to colleagues, providing advice and allocating time to each practice.
Effectively managing a varied caseload of clients, ensuring that all quality and monitoring standards are met and that performance information is provided within the required timescales.
Following up patients who have been referred for a 2 week wait cancer referral to ensure that they have support in place and are helped to manage any risk factors they may have.
Working with patients and their families with identified safeguarding issues to identify and address their needs

MWEB PCN is a Primary Care Network comprising of Moulton Surgery, Woodview Medical Centre, Kingsthorpe Medical Centre, Denton Village Surgery and Wootton Medical Centre.
We provide services to over 40000 patients in and around Northampton.

Our Social Prescribing Link Workers support a wide range of patients, both those referred in by GPs, Nurses and other practice staff as well as people who request support.
The patients are at the heart of our service. We give them time to talk and explore the non-medical issues that are impacting their wellbeing, including the wider determinants of health such as housing, finance and employment.
We use solution focused techniques such a motivational interviewing to help people identify what matters most to them, before coproducing a simple action plan.
With this plan in place, we use our community knowledge to link people to the most appropriate services, supporting them to access them and to move towards independence.

Person Specification
Experience Essential
Experience of working with resilience with people who have care or support needs either in a paid or voluntary capacity Excellent communication and interpersonal skills which can be tailored to meet the needs of different audiences Ability to forge and develop excellent partnership working practices across a wide range of sectors and disciplines Ability to understand peoples diverse and complex needs and use a strengths based approach to meet these needs by linking them to other services Ability to work independently within a target-driven and time-limited system, dealing with issues pragmatically and through brief interventions A commitment to improving peoples wellbeing through practical community based interventions Ability to work flexibly and independently flexing in relation to peoples demands and the requirements of a new and developing team A current UK driving license and access to a vehicle Excellent IT skills Ability to effectively manage a varied caseload, creating action plans, providing support and working towards independence A knowledge of social prescribing and its benefits to recipients, the NHS and the wider community Experience and confidence using strengths based assessments, support planning and motivational interviewing techniques
