Registered General Nurse - Focus on Living Centre
Part-Time 18 hours per week
£35,420 - £38,085 per annum full time which is £17,001 - £18,280 pro rata for 18 hours per week
To be worked over Tuesday-Thursday
This is a fixed term contract for 2 years
St Oswalds Hospice is a renowned hospice dedicated to providing compassionate end-of-life care to individuals and their families. Our mission is to provide specialist palliative and end of life care to adults, children and young people in the north east of England.
An exciting opportunity has arisen for an experienced and enthusiastic Registered General Nurse to work alongside and support the Sister of the Focus on Living Nursing Team in delivering high-quality care to people living with progressive, life-limiting illnesses.
The Focus on Living Centre is an evolving and innovative service providing holistic palliative care through a combination of a Virtual/Day Ward and therapeutic wellbeing programmes.

Main duties of the job
Working as part of a multidisciplinary team, the service supports people with complex palliative and supportive care needs through clinical assessment, symptom management, care coordination, rehabilitation, and wellbeing interventions delivered both face-to-face and virtually.
Our services include the assessment, management and coordination of the Virtual/Day Ward, therapeutic and wellbeing activities, virtual groups, telephone support, and face-to-face consultations.
The successful candidate will play a key role in the delivery and ongoing development of the Virtual/Day Ward, supporting patients to remain safely at home while receiving high-quality specialist palliative care. The role includes monitoring patient needs, developing care plans, coordinating with community and acute services, and contributing to multidisciplinary decision-making.

To provide exceptional nursing care and support to patients with progressive life-limiting conditions within the Focus on Living Centre. Working autonomously and as part of a multidisciplinary team, the Staff Nurse will play a key role in the assessment, coordination and delivery of care within the Focus on Living Virtual/Day Ward, supporting patients with complex palliative and supportive care needs to remain safely at home where appropriate.
With a commitment to holistic care, the role includes the management of a caseload of patients, supporting advanced care planning and therapeutic interventions, ensuring patients and their families receive comprehensive, empathetic, and person-centred care.
