We have an exciting opportunity for a Transfer and Discharge Coordinator to join our Substance Misuse team at HMP Lewes, a category B Prison located in Lewes.

HMP Lewes is a busy male prison with a capacity for 742 inmates. Hours

This role is a fixed term contract for 12 months.

Monday - Friday

0800 - 1600 Salary

This position is full time, youll receive an annual salary of £29,000 per annum.

Main duties of the job

As a Transfer and Discharge Coordinator, your responsibilities will vary. You will

Reducing unnecessary and confusing handoffs between healthcare services and inconsistency in service transfer between healthcare providers by initiating early discharge planning or transfer to alternative settings. This will include transitioning existing care plans.

Ensuring safe transfers between establishments which offer continuity of care and ongoing support.

Ensuring patients goals and needs are taking into consideration when care planning prior to transfer, with appropriate actions put in place for patients with complex heath care needs.

To remove the delay for patients accessing and receiving the healthcare and medication they need during the transfer process.

Work alongside the recovery team to promote and improve continuity of patient care.

Meeting with patients to review recovery needs

Making links with community drug & alcohol team

Ensure patient's needs are going to be met by the receiving team e.g. OST prescription, appointments in the right place, help link prison and community team

Qualifications, skills and experience required:

Experience in working with patients within the Criminal Justice System

Evidence of implementation of local and national policies e.g. NICE guidance, developing protocols/care pathways and monitoring effectiveness

Knowledge of audit and its relationship to outcomes

Knowledge of the statutory requirements for the safeguarding of adults and children

Willing to be innovative and embrace learning through continued professional development and the practical application of new skills in the workplace

Excellent communication skills and ability to communicate openly, consistently and clearly

A non-judgmental, patient centred and compassionate approach
