This is by supporting the multi-professional team in the assessment and implementation of falls prevention treatment programmes and care plans for patients who may have diverse presentations and varying physical and psychological conditions. Agreed treatment programmes / care plans will be carried out by the post holder, at times, working alone within the patients home.
Main duties of the job Key Responsibilities: 1. To carry out prescribed treatment programmes and care plans with patients including those with diverse or complex disabilities and multi pathologies, seeking assistance from a qualified member of the Waltham Forest Falls Prevention Service as required. 2. To monitor treatment / care plans and evaluate patient progress to a satisfactory outcome, reporting on and advising the Registered Therapist/Band 4 of any changes, problems and outcomes, or requesting assistance and advice as required. 3. Assist the Registered Occupational Therapists, or Physiotherapists in home visit assessments/treatments and carry out lone access home visit assessments/treatments as directed. 4. Working in the community and domiciliary settings work as a lone practitioner with instruction and support from an Occupational Therapist, Physiotherapist, Nurse or Band 4. 5. To be aware of and responsible for reporting clinical risk within own patient caseload. 6. Work within trust clinical guidelines and professional guidelines and to have a good working knowledge of national and local standards and monitor own and others quality of practice as appropriate. 7. Represent the Waltham Forest Falls Prevention Service as instructed. To liaise with members of the multi-disciplinary team, under the guidance of the responsible therapist/nurse.

We are looking for 2 Rehabilitation Assistances to work in the Falls Prevention Service- one full time (37.5 hours a week) and one part time (24.75 hours a week)
The post holder will be a Rehabilitation Assistant (Band 3), assisting Waltham Forest Falls Prevention Service in providing a high standard therapy service to patients within the community.
This is by supporting the multi-professional team in the assessment and implementation of falls prevention treatment programmes and care plans for patients who may have diverse presentations and varying physical and psychological conditions.
Agreed treatment programmes / care plans will be carried out by the post holder, at times, working alone within the patients’ home. They will also assist in the delivery of Falls Education and Balance Classes.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Hospital.

High Cost Area Supplement – Outer London (B&D, Redbridge, Havering and Waltham Forest, CEME) This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,870 to a maximum of £6,137 p.a. pro rata for part time).
