We pride ourselves on delivering a patient-centred service and fostering a supportive and friendly working environment for our staff.We are seeking a motivated and organised Care Coordinator to join our team. The successful candidate will play a key role in supporting patients and clinicians by coordinating care, improving communication between services, and helping patients navigate the healthcare system. Working closely with GPs, nurses, healthcare professionals, and external agencies, you will help ensure patients receive the right care, at the right time, from the right service.

Main duties of the job

Coordinate care for patients with long-term conditions, frailty, and complex health needs. Support patients to access appropriate health and community services. Liaise with hospitals, community teams, social care, care homes, and voluntary organisations. Assist with monitoring and tracking patient pathways and outcomes. Support multidisciplinary team meetings and follow up agreed actions. Help identify patients who may benefit from additional support or targeted interventions. Maintain accurate and up-to-date patient records. Provide excellent patient service through telephone and written communication.

Person Specification

Experience Essential

We are looking for someone who - Has excellent communication and interpersonal skills. Is highly organised and able to manage multiple priorities. Has experience of working in healthcare, social care, or a customer-focused environment. Demonstrates empathy, professionalism, and a patient-centred approach. Is confident using IT systems and databases. Can work independently and as part of a multidisciplinary team.

Desirable

Experience within primary care or the NHS would be advantageous but is not essential, as training will be provided for the right candidate.
